Aortic valve-sparing surgery. Our experience and medium-term outcomes

The aim of our study was to determine the medium-term outcomes of aortic valve-sparing surgery. A total of 37 such procedures were performed in our centre during the 2002-2009 period. The procedure was indicated for chronic ascending aortic aneurysm in 23 cases, acute dissection in 13, and type A chronic dissection in one case. The follow-up period was 1-5 years. Four patients (10.8%) required re-do surgery for significant aortic regurgitation.

The authors present their experience with the use of splint osteosynthesis in the treatment of complicated sternotomy wounds with bone dehiscence due to wire cerclage failure. Two systems for the reconstruction and stable internal fixation of the sternum used by our team and the technique of implantation are mentioned briefly. Discussion outlines the advantages and disadvantages of this relatively new method of stabilization of the sternum the use of which has been widespread in cardiac surgery practice.

We present the case of a female patient after left-side mastectomy for carcinoma with subsequent actinotherapy, who had a myocardial revascularization procedure using bilateral internal mammary artery harvest.Wound healing was complicated with deep sternal infection with three attempts at closing the dehiscence by soft tissue repair until complete healing. Aim of the study: To evaluate short-term and long-term results of surgical treatment of infective endocarditis (IE) in patients operated on in our department.Methods: From 1994 to 2007, 63 patients with the diagnosis of infective endocarditis underwent operation in Motol University Hospital. Aim: To evaluate outcome of surgery of the tricuspid valve only.Patients and methods: Between 1998 and 2008, isolated tricuspid valve surgery was performed in 54 patients (8.9% of all patients undergoing combined procedures including the tricuspid valve). In cardiac surgery, demand for blood transfusion, while historically variable, has been generally high. During the 50 years of evolution of cardiac surgery, average use of transfusions has decreased significantly, but there has been a trend toward a rise in the last decade. The causes are mainly the increasing number of reoperations and procedures in elderly, polymorbid patients with bleeding complications. Cardiac tamponade caused by right atrial destruction due to pleural mesothelioma metastasis

This report describes the case of a 51-year-old patient who was referred with the diagnosis of pericardial tamponade from another hospital. After drainage of 1900 cc of hemorrhagic exsudate, the suspicion of aortic dissection was ruled out by a subsequent CT angiogram and the source of bleeding was identified in perforation of the right atrium. The patient was operated acutely, tamponade was relieved and atrial perforation was sutured. An nfiltrating cardiac tumor was identified as the cause of the perforation. Histology revealed pleural mesothelioma metastasis.

Caseous calcification of the mitral annulus is a rare form of mitral annular calcification which may often cause diagnostic quandary. We report the case of a 69-year-old man scheduled for cardiac surgery on suspicion of a malignant process involving the posterior mitral annulus and encroaching upon the left atrium and left ventricular myocardium. Tricuspid valve endocarditis in a drug addict: will two operations do?

A case of tricuspid valve endocarditis in a young drug addict is described which was treated surgically by valve replacement. Failure to stop intravenous drug abuse led to early destruction of the implanted bioprosthesis in a new onset of prosthetic valve endocarditis. A comparison of the operative findings at the initial operation and reoperation underlines the difficulties in the treatment of infective endocarditis in intravenous drug addicts. Different treatment strategies are discussed.
