Blood bank transfusion and blood salvation in cardiac surgery

In cardiac surgery, demand for blood transfusion, while historically variable, has been generally high. During the 50 years of evolution of cardiac surgery, average use of transfusions has decreased significantly, but there has been a trend toward a rise in the last decade. The causes are mainly the increasing number of reoperations and procedures in elderly, polymorbid patients with bleeding complications. Our approach involves a multimodal policy of patient's blood salvation extending from preoperative preparation, through surgery and recuperation procedures up to homeostasis maintenance. Despite numerous problems, blood bank transfusion remains a life saving step. Our current blood conservation policy allows for cardiac surgery without blood bank transfusion (bloodless procedure) in 60% patients and further improvement can be expected.
