Heart failure and atrial fibrillation are often referred to as the epidemics of the 21st century. They are conditions characterized by an appreciably increasing incidence with increasing age - the typical patient is over 70 years old. The prerequisite for successful drug therapy of both conditions is effective oral anticoagulation and an individualized choice between rhythm versus rate control in an effort to prevent sudden cardiac death or progression of heart failure. Some aspects of the effect of the marathon race on the cardiovascular system

Multiple studies have documented transient cardiac dysfunction and provided biochemical evidence of cardiac injury (release of biomarkers) after finishing a marathon race. This case report describes a 43-year-old amateur marathon runner who finished a marathon race and subsequent echocardiographic examination revealed mild left ventricular diastolic dysfunction. Discussion is focused on the transient myocardial dysfunction and release of biomarkers after the completion of a marathon race.

Given the imbalance between the offer of and demand for donor hearts, heart retransplantation is a controversial "procedure"; on the other hand, retransplantation is often the only therapeutic option in the presence of heart graft failure. We report the case of a patient in whom even repeat endomyocardial biopsies failed to identify severe acute cellular rejection as the cause of heart graft failure.

This case report raises the question whether abdominal ultrasound belongs to standard diagnostic methods in patients with right heart failure of unknown etiology.A 79-year-old man was admitted to hospital because of progressive dyspnea and symptoms of right heart failure. Echocardiography demonstrated preserved left ventricular systolic function and pulmonary hypertension. The patient died on day 9 of hospitalization of fulminant right heart failure.
