The value of pressure gradient measurement in iliac artery angioplasty

Unusual location of the substrate of fascicular idiopathic left ventricular tachycardia

A successful ablation of fascicular left ventricular tachycardia located on a false tendon in the left ventricle is presented. A high-frequency His-Purkinje potential was recorded along the fascicles as well as around the insertion and along the false tendon in sinus rhythm. The position of the mapping catheter was monitored by echocardiography. During tachycardia, early endocardial activation with an identical ECG pace map was recorded at the septum in the area of insertion, however, repeated attempts to ablate the tachycardia in this region failed. Temporary cyclosporin withdrawal in a patient after heart transplantation during chemotherapy for post-transplant lymphoproliferative disease

A patient undergoing orthotopic heart transplantation, performed in 1986, had post-transplant lymphoproliferative disease with two relapses. The disease became manifest in 1995, 9 years after surgery, with the first and second relapses occurring in 1999 and 2001, respectively. First-line therapy included CHOP combination therapy, to be replaced by the more aggressive Pro MACE Cyta BOM protocol after the first relapse. The second course of chemotherapy was associated with cyclosporin withdrawal. No signs of rejection were identified in any of the biopsy specimens obtained from the patient.

Aortic valve replacement with a bioprosthesis is currently indicated in elderly individuals. Patients with a bioprosthesis do not require anticoagulation therapy. The advantages of stentless prostheses include a low transvalvular gradient, especially so in patients with a narrow aortic annulus, while the drawbacks include more complex implantation. A new model of prosthesis, the Shelhigh SuperStentless, allows standard safe single-layer implantation. The authors describe their favorable early results in 30 patients operated on for severe aortic stenosis.
Valve involvement during treatment with anorectic agents: echocardiographic follow-up

Using echocardiography, valvular structural changes, demonstrated by other authors after long-term treatment, were assessed in a series of our 28 patients treated with serotoninergic anorectic agents. Major valvular regurgitation was not observed in any of our patients. This is most likely to be due to the generally lower dose of the anorectic and shorter duration of therapy compared with large groups evaluated in international studies.
