Introduction: Polymorphisms in the genes encoding the cytochrome P 450 2C9 enzyme and the vitamin K epoxide reductase, subunit 1 (VKORC1) are known to contribute to the sensitivity to warfarin.Aim of the study: We wanted to determine the prevalence of CYP2C9 (allele *1, *2 and *3) and VKORC1 (-1639 G and A alleles), and their combinations in Czech population to get an idea of how many percent of individuals can theoretically involve increased sensitivity to warfarin. Methods: Genotyping was performed in 1300 healthy subjects (774 men and 526 women) using a robotic DNA isolation and subsequent PCR amplification according to the manufacturer's...

Cor Vasa 2011, 53(10):527-534 | DOI: 10.33678/cor.2011.132
Objective: Statins are one of the most prescribed medications in our country. About 2 billions CZK are being paid for them annually in the Czech Republic (CR). Objective information about to whom, why, how, by whom and with what results these medications are given is, however, missing. It was the aim of the STEP project (Statin Therapy REsults in the Real World Practice in the Czech Republic), which was carried out in the CR in 2010, to obtain the missing information.Patients and methods: In this non-intervention retrospective survey, data from 3190 patients treated by statins for at least 12 months in specialized outpatient clinics were collected...
Are our criteria for selection of candidates for cardiac resynchronization therapy correct?

Cor Vasa 2011, 53(10):535-541 | DOI: 10.33678/cor.2011.133
Introduction: A number of studies have documented the effect of cardiac resynchronization therapy (CRT) on improving functional status and inducing reverse left ventricular remodeling. Using current indication criteria, the proportion of non-responders, i.e., those not responding to CRT, has been reported to be 30-40%.Aim: To assess, in patients with CRT devices, the effect of CRT on changes in NYHA class and echocardiographic parameters, to assess the effect of heart failure etiology and baseline rhythm, and to determine the ability of QRS width and ventricular dyssynchrony parameters to predict the effect of CRT.Patients and methods: A total...

Aim: To evaluate short- to mid-term outcomes of aortic valve sparing operations performed at our hospital. Methods: In the period from January 2008 to August 2010, the total of aortic root reconstructions including aortic valve sparing operations were performed at the Clinic of Cardiac Surgery, University Hospital UPJŠ, Košice. Only 15 patients were included in the follow-up, one patient died in an early postoperative period and one patient required early reoperation with aortic valve replacement. The average age of the patients was 55 ± 9 years. The average grade of aortic regurgitation before the surgery was 2.44 ± 1.2. Dilatation...

Introduction: Treatment with long-acting antihypertensive medication prevents the blood pressure elevation in morning hours, thus decreasing the cardiovascular risk. We used ABPM to assess the effect of imidapril on 24-hour blood pressure control in a 10-week trial.Materials and methods: 412 patients (52% men) aged > 18 years (21-89; mean: 58; median: 60) with mild to moderate hypertension were enrolled. Imidapril monotherapy was used in 302 patients (73%) (de novo treatment or switch from another), 65 patients (16%) received combination of imidapril with another antihypertensive drug and not enough data for assessment were available from 45...

Refractory hypertension carries high risk of complications. Renal denervation (RDN) is a new invasive method of catheter-based radiofrequency ablation of the sympathetic nerves located around renal arteries. The initial clinical experience showed promising and long (2 years) lasting decreases in blood pressure. RDN has provided interesting results also in patients with hypertension plus heart failure and among diabetics with hypertension. This article reviews the published data and provides insight into the methodology of RDN.

Closure of the left coronary artery orifice by a cusp of quadricuspid aortic valve with aortic insufficiency
Věra Pečenková, Tomáš Marek, Vladimír Karmazín, Jan Pirk, Tomáš Miřejovský, Josef Kautzner
Cor Vasa 2011, 53(10):552-554 | DOI: 10.33678/cor.2011.136
Quadricuspid aortic valve is a rare inborn anomaly, first described in 1862. The authors present a case of 58-year-old man with an insufficient quadricuspid aortic valve, closure of the left coronary artery orifice by one of its cusps and an anomalous myocardial blood supply through collateral vessels. Exertion ischemia manifesting by left-sided cardiac decompensation was demonstrated in this patient.
Tako-tsubo cardiomyopathy with both ventricles involvement

Cor Vasa 2011, 53(10):555-558 | DOI: 10.33678/cor.2011.137
The authors present a case of a female with tako-tsubo cardiomyopathy with the left and right ventricular involvement. Regional wall motion abnormalities of the right ventricle occur in approximately 25-30% of patients with tako-tsubo cardiomyopathy and have a similar character and localization as in the left ventricle. The right ventricular involvement is frequently associated with a greater reduction in the left ventricular systolic function and seems to be a possible marker of worse course of the disease during the acute phase due to higher frequency of complications and need for a longer duration of hospitalization.
Historical and current clinical usage of verapamil, beta-blockers and their mutual combinations in arrhythmology

Review of combinations of verapamil, bisoprolol and moxonidine in 22-year-old hypertensive man is presented, and significant sympto-matic pro-arrhythmogenic effect of this treatment is mentioned. Whole range of secondary - iatrogenic - arrhythmias is described in this case report. Disordered impulse formation and conduction, activation of alternate pacemakers and ectopic pacemakers were all seen in this patient. The observed dysrhythmias were fully reversible, with a restitution of sinus rhythm within 48 hours of pro-arrhythmogenic antihypertensives discontinuation. Therapeutic use of the combination of verapamil and beta-blockers in arrhythmology...
PTFE prosthesis interposition as a treatment of intraoperative injury to the arteria carotis interna
Bohumil Horký
Cor Vasa 2011, 53(10):563-565 | DOI: 10.33678/cor.2011.139
We present a case report concerning an operation for simultaneous stenosis and kinking syndrome of the internal carotid artery (ICA) where iatrogenic injury to the internal carotid artery took place. The resulting urgent situation was solved with PTFE prosthesis interposition. This solution was chosen as a method of first choice because of the time pressure where autologous material harvesting would inappropriately prolong the time of the ICA laceration repair.

Cor Vasa 2011, 53(10):566-568 | DOI: 10.33678/cor.2011.140
This case report presents 56-year-old woman after successful cardiopulmonary resuscitation for ventricular fibrillation. Diagnosis of anterior myocardial infarction with primary ventricular fibrillation was determined based on ECG and echocardiographic images. Coronary angiography, repeated detailed echocardiographic examination and magnetic resonance imaging, that were performed later, disclosed hypertrophic cardiomyopathy with apical aneurysm, which is less common for this diagnosis. Implantable cardioverter defibrillator (ICD) was implanted in secondary prevention of sudden cardiac death. We found several adequate ICD therapies in one year follow-up....

Cor Vasa 2011, 53(10):569-571 | DOI: 10.33678/cor.2011.141
We report two cases of patients with a complete left bundle branch block (LBBB) with similar cardiac symptoms but different outcomes due to myocardial perfusion and function imaging, using single photon emission computed tomography (SPECT).
