Closure of the left coronary artery orifice by a cusp of quadricuspid aortic valve with aortic insufficiency

Quadricuspid aortic valve is a rare inborn anomaly, first described in 1862. The authors present a case of 58-year-old man with an insufficient quadricuspid aortic valve, closure of the left coronary artery orifice by one of its cusps and an anomalous myocardial blood supply through collateral vessels. Exertion ischemia manifesting by left-sided cardiac decompensation was demonstrated in this patient.
