Aliskiren is the first oral renin inhibitor. In healthy individuals, single and multiple doses of aliskiren result in peak plasma levels within 3-6 hours and the mean biological half-life after multiple doses is 40.7 ± 10.7 hours. It is just this long half-life which makes it possible to administer the drug once daily and ensures good blood pressure control even in the morning. It is eliminated mainly unchanged in feces. Only a small proportion (12%) of aliskiren is excreted during first-pass liver metabolism in the liver, with less than 10% of aliskiren excreted via the kidneys. The long QT syndrome is characterized by QT interval prolongation and increased risk of malignant arrhythmias, which can cause syncope or even sudden death. A typical feature is absence of structural cardiac disease. The article summarizes the most recent information on the syndrome "from cell to bedside": pathophysiology at molecular and cellular levels, clinical diagnosis and the current recommended therapeutic approaches. Lyme carditis is the term used for cardiac involvement caused by infection by the spirochete Borrelia burgdorferi. The disease manifestations include mostly arrhythmias, with pericarditis or myocardial dysfunction being less common. The authors describe a case of Lyme cardiomyopathy with severe left ventricular systolic dysfunction. The final diagnosis was based solely on endomyocardial biopsy as the serological examination did not reveal any presence of specific antibodies against Borrelia. Infectious endocarditis of the mitral valve following injury by a pet

Infectious endocarditis (IE) is a serious intracardiac infection, deadly in 100% of cases in the preantibiotic era. The introduction of penicillin and streptomycin into the therapeutic protocols was followed by a steep decrease in IE-related mortality to levels below 20%. Today, despite the advances in the diagnosis and treatment, IE continues to be a disease with an uncertain prognosis and total mortality in the range of 20-30%. In her case report, the author presents an unusual cause of IE, highlighting the need for multiple clinical examinations of at-risk individuals in a septic state.
