Introduction: Hypoplastic left heart syndrome (HLHS), one of the most serious congenital heart defects, can be surgically paliated using 3 subsequent stages. Long-term results of this approach are reviewed in this study.Patients: Fifty-two consecutive patients (pts.) operated for HLHS from 1999 to 2012 were evaluated retrospectively. Methods and results: Norwood stage I operation was performed at the median age of 6.5 days with a total mortality of 19%. Significant risk factor for death was lower weight at surgery (Cox proportional risk per 1 g increase = 0.997, CI 0.995-0.990, p < 0.001). Between stages I and II, 15 catheter/surgical reintervention...

The findings of the oesophageal echocardiography in patients with acute cerebral ischaemia

Cor Vasa 2014, 56(6):e456-e462 | DOI: 10.1016/j.crvasa.2014.07.001

Introduction: The cerebrovascular accident ranks among the main causes of mortality and morbidity worldwide. In the Czech Republic, the cerebrovascular disease was the direct cause of the death in 10% of the cases of death in the year 2012 (Healthcare Annual CZ 2012).Aim: The aim of our study was to evaluate the transoesophageal echocardiogram findings (TOE) in patients with suspicion for cardioembolic stroke or TIA who were indicated to this examination in our echocardiography laboratory.Methodology: During the period from July 2009 to March 2013 (45 months), 108 TOE had been realized in our echocardiographic laboratory that was indicated...

High-dose spironolactone changes renin and aldosterone levels in acutely decompensated heart failure

Cor Vasa 2014, 56(6):e463-e470 | DOI: 10.1016/j.crvasa.2014.06.004

Background: In acutely decompensated heart failure (ADHF) patients higher aldosterone levels correlate with worse post-discharge outcomes, suggesting that further modulation of the mineralocorticoid system during or immediately after hospitalization might favourably improve outcomes. Methods and results: This was an observational, retrospective secondary analysis of a study including 100 patients with ADHF. In that study 50 patients were submitted to spironolactone treatment (50-100 mg/day). A higher proportion of patients with renin levels above 16.5 pg/mL and aldosterone levels above 100 ng/dL was observed in subjects submitted to spironolactone...

Cor Vasa 2014, 56(6):e471-e477 | DOI: 10.1016/j.crvasa.2014.05.004

The study aimed at assessing the rotational motion of the left ventricle around the long axis in patients affected by isolated left ventricular noncompaction (LVNC) and comparing their results with those of healthy volunteers.Patients and methods: The study comprised eight patients with LVNC confirmed by echocardiography and magnetic resonance imaging (mean age 41 ± 19 years; four males; left ventricular ejection fraction [LVEF] 45 ± 25%). The patients were divided into Group A with an LVEF above 50% (four patients; one male) and Group B with an LVEF below 50% (four patients; three males). For both groups, age- and sex-matched volunteers...

Introduction: Coronary bifurcation lesions account for 15-20% of all percutaneous coronary interventions. Dedicated bifurcation stents have recently been introduced with the aim to simplify treatment and improve early and late outcomes following stenting of bifurcation lesions. The purpose of our study was to assess the safety and effectiveness of the Tryton dedicated side branch stent at a 6-month clinical and angiography follow-up.Methods: Forty-two patients with bifurcation lesions were included in our study. The primary endpoint was a 6-month MACE and angiographic stent patency was also evaluated by MS-CT coronarography. Results: Twenty-two...

Past, present and future of cardiovascular twin studies

The increasing burden of cardiovascular disorders highlights the crucial need to research efforts in the prevention and screening of heritable disorders or cardiovascular risk factors. Twin studies offer an opportunity to assess the influence of genetic and environmental factors and gene-environment interactions as well. Blood pressure and its components, arterial stiffness, carotid intima-media thickness were found to have a moderate heritability, whereas carotid plaque formation and coronary atherosclerosis seemed to be strongly genetically determinated. These findings so far underlined the importance of identification of the specific genetic factors...

Right atrial diverticulum associated with the Wolff-Parkinson-White syndrome in a child

Cor Vasa 2014, 56(6):e519-e522 | DOI: 10.1016/j.crvasa.2014.07.003

A child with Wolff-Parkinson-White syndrome showed large right atrial diverticulum overlying the right ventricle and the right coronary artery at CT angio with right atrial appendage as a separate structure. The accessory pathway was located at the diverticulum. Catheter ablation was not regarded feasible and surgery was indicated. A broad muscular connection between the right atrium and ventricle was interrupted by transsection and cryoablation as confirmed by disappearance of preexcitation. This precisely CT documented case represents a rarely reported right atrial diverticulum posing a high risk of life-threatening arrhythmias.

Aneurysms of the transverse aortic arch requiring surgery most often affect elderly patients with multiple co-morbidities and represent a significant challenge to both patient and surgeon. The hybrid approach developed in recent years (debranching followed by endovascular repair) may improve the morbidity and mortality of the population risk. We present a case-report of 72-year-old man with aortic arch aneurysm arising at the origin of the left subclavian artery involving whole caudal segment of an aortic arch with concomitant single vessel coronary disease. The hybrid procedure was carried out in two stages, first (open surgical approach) performing...
