Aortic valve replacement in patients over 80 years of age

Aim: To evaluate the outcomes of aortic valve replacement in patients over 80 years of age.Methods: In the 1997-2007 period, a total of 91 patients aged over 80 years had surgery at the Department of Cardiac Surgery of the Institute for Clinical and Experimental Medicine. Both early (30-day) and late mortality of these patients undergoing aortic valve replacement was assessed. Using statistical methods, an attempt was made to identify risk factors involved in early and late mortality and, also, independent risk factors for late mortality.Results: Early and late mortality was 6.6% and 28.6%, respectively, with a mean follow-up period of 33 months....

Background: Primary sternal closure after surgery for congenital heart defect is not feasible in all newborns because of tissue swelling, cardiomegaly, or postoperative bleeding. Infectious endocarditis remains to be a serious, life-threatening condition even today. Mortality is increased, exceeding 30%, with endocarditis involving prosthetic valves. The authors present the case of prosthetic valve endocarditis involving the aortic valve prosthesis with embolic complications resulting in deterioration of the patient's condition and, eventually, his death.

The paper presents the case report of a young male patient with severe systemic hypertension caused by increased intracranial blood pressure due to a large prolactinoma. The patient experienced sight impairment due to local oppression by the tumor, had left ventricular hypertrophy and hypertension-induced renal failure. Anti-edema therapy was associated with partial remission of the presentations of intracranial hypertension. Use of trimetazidine in the treatment of cardiac ischemia

Trimetazidine improves the metabolism of the ischemic myocardium at the cellular level. In patients with stable angina, it reduces the frequency of anginal attacks and nitrate consumption while increasing exercise tolerance. Because of trimetazidine's additive action, it can be combined with advantage with other antianginal agents. As the agent has no effects on hemodynamic parameters, undesirable effects are negligible. Regarding other indications, no decisions can be made until data from large trials have become available.
