Interventional treatment of the elderly with acute coronary syndrome is a complex, mostly interdisciplinary problem that we are faced with increasingly often due to the aging of the population. Although the interventional technique is the same as in the younger population, we have to count with a primary, much higher global and cardiac risk in the elderly. Non-ST-elevation acute myocardial infarction (NSTEMI) is a serious disease with different clinical, angiographic and laboratory findings. The name of the Czech session at the largest European congress of interventional cardiology, EuroPCR 2008, "Are all non-ST-elevation acute myocardial infarction patients equal?" pointed out very well the differences among the patients. The session prepared by the Working Group of Interventional Cardiology of the Czech Society of Cardio-logy consisted of three interesting cases followed by short reviews of up-to-date evidence. This article is a short summary of the presentations focusing on some of the "hidden" problems.
Spontaneous coronary artery dissection

Spontaneous coronary artery dissection (SCAD) is a rare cause of acute coronary syndrome in young patients without apparent risk factors for coronary artery disease. It is predominantly young healthy females who are affected during pregnancy and the peripartum period, with connective tissue diseases or autoimmune diseases implicated in the pathophysiology of this condition. Therapeutic approaches include conservative medical therapy, coronary artery bypass graft surgery, and percutaneous coronary intervention (PCI) with intravascular ultrasound guidance, depending on the extent of disease and clinical symptoms.

Non-traditional management of left main stenosis presenting as NSTEMI in an elderly female patient with severe coronary artery calcifications

The author presents the case report of a 84-year-old lady with non-ST elevation myocardial infarction due to left main stenosis treated by percutaneous coronary intervention with rotational atherectomy and "bare" metal stent implantation using the "V" technique.
Acute myocardial infarction in patients after coronary artery bypass grafting

Acute coronary syndrome in patients after coronary artery bypass grafting is caused mostly by unstable lesion in the saphenous vein graft, less often by unstable lesion in the native coronary artery. We report a case, in whom it is difficult to decide whether to perform percutaneous coronary intervention in unstable lesion of the saphenous vein graft or in chronic stable lesion in the native coronary artery supplied by this graft.
