Introduction:Syncope is a relatively serious symptom in patients with atrial fibrillation (AF). Aside from arrhythmic syncopes, there are neurocardiogenic syncopes (NCSs), based on the autonomic nervous system s reflex mechanism, in patients with AF. Almost as a rule, interventricular septal defect poses a fatal complication during the course of acute myocardial infarction. In the acute phase, it is managed by surgery; related problems include timing of the procedure, high perioperative mortality, and the risk of defect recanalization.We present the case report of a 55-year-old man with his first myocardial infarction in the presence of single-vessel disease, complicated by interventricular septal rupture. This syndrome encompasses at least four diagnostic entities: cardiogenic shock, pulmonary edema, hypertensive crisis, and acute exacerbation of heart failure.
