Cor Vasa 2002, 43(7-8):319-323

Introduction:Catheter radiofrequency ablation (RFA) has become, in the last decade, a rapidly developing method of non-pharmacological treatment of cardiac arrhythmias. In recent years, it has become the treatment of first choice in typical atrial flutter, with anatomically controlled line of RF energy delivery in the region of the cavotricuspid bridge. The risk for thromboembolic complications in atrial flutter has long been rather underestimated, it was not until recently that several studies have demonstrated an increased incidence of thromboembolism, especially so in connection with cardioversion. Method and group of patients:Our...

The TIGER study. The efficacy and tolerability of trimetazidine in geriatric patients exertion angina pectoris

František Kölbel, Viliam Bada

Cor Vasa 2002, 43(7-8):325-330

Aim of study:To evaluate the efficacy and tolerability of the myocardial energy metabolism modulator trimetazidine (TMZ), a 3-ketoacyl-coenzyme A thiolase (3-KAT) inhibitor in elderly patients with stable exertion angina pectoris. Method:A total of 120 patients aged 65-86 years (mean 70 ± 4 years) with chronic stable exertion angina pectoris were included into the study. Ninety of these individuals (75%) had a history of myocardial infarction. Exercise testing was performed twice within a week prior to inclusion (T-1, T0); next, oral TMZ at a dose of 20 mg three times a day was added to therapy stable for at least 4...

Cor Vasa 2002, 43(7-8):332-335

Introduction:Acromegaly is a disorder associated with increased cardiovascular morbidity and mortality rates. The aim of our study was to compare parameters of vascular function and morphology and establish their relationship to some biochemical variables. Methods:Twenty-two patients with hormonally active acromegaly and fifteen healthy controls were included in the study. The examination was performed in the morning after an overnight fast. The participants underwent clinical examination, measurement of IMT and laser-Doppler examination of microcirculation; blood samples were taken for laboratory analysis. Results:We...

Treatment of patients with acute myocardial infarction eligible for thrombolysis

Cor Vasa 2002, 43(7-8):336-343

Aim:To determine the number of patients with acute myocardial infarction (AMI) whose medical history and EKG allow to establish an early diagnosis of AMI and to analyze data, in AMI patients eligible for thrombolytic therapy (TLT), on TLT rates and hospital delay. Patients and methods:The project was undertaken as a prospective multicentric study. Independent audit and data collection were undertaken in a total of 3,123 AMI patients in 66 centers in the period from 16 September 1997 through 15 September 1998. Results:Timely diagnosis of AMI (still before detecting the serum markers of myocardial necrosis) could...

We present a port-access way to create a sutureless proximal anastomosis on the ascending aorta using the Symmetry™ Aortic Connector System (St. Jude Medical, Inc., St. Paul, MN, USA). This technique was successfully used in a patient undergoing urgent minimally invasive direct coronary artery bypass (MIDCAB) whose left internal mammary artery could not be harvested due to subtotal occlusion of the left subclavian artery. Port-access vein graft deployment expands the potential of "less invasive" coronary surgery. This case has demonstrated the potential benefits of sutureless anastomosis, the suitability of minimal access approach.

Extra-anatomical bypass with Bentall's procedure in the treatment of complicated recoarctation of the aorta

Cor Vasa 2002, 43(7-8):349-351

In their case report, the author present a patient with complicated recoarctation of the aorta managed by extra-anatomical bypass from the ascending aorta to the descending thoracic aorta. The bypass was performed simultaneously with Bentall's procedure from midline sternotomy. The posterior pericardial approach was used to gain access to the descending aorta. The indication for the procedure was recoarctation of the aorta in a patient with ascending aorta aneurysm and aortic valve regurgitation.Despite a complicated early postoperative course, the patient was discharged after 2 months to receive home care. Aortic angiography performed at 16...

Cor Vasa 2002, 43(7-8):353-355

The aim of the study is to present initial experience with transmyocardial laser revascularization (TMLR) using the LUXCOR 500/800, a new CO2 laser device. Our initial experience is based on surgery performed in 13 patients. TMLR resulted in a reduction in angina pectoris in almost all patients. The actual procedure is simple, led to no postoperative complications, and there were no deaths.
