Introduction: Hypertension represents the major risk factor for cardiovascular diseases. Pre-hypertensives are more likely to progress to hypertension. The mechanism for the progression of pre-hypertension to hypertension remains unclear. Hence the present study was designed to investigate the association of endothelin-1 with oxidative stress and inflammatory response in pre-hypertensives. Methods: The study was conducted in thirty subjects with pre-hypertension and thirty normotensive subjects. Lipid profile, lipid bound sialic acid (LBSA), malondialdehyde (MDA) and endothelin-1 (ET-1) levels were estimated in the fasting blood samples. Results: Total...

Background: Chronic coronary total occlusions (CTOs) are mostly associated with infarct-related artery (IRA CTO); however, all CTOs may not always lead to or be caused by STEMI. Clinical profile of patients with non-infarct-related artery related CTOs (non-IRA CTO) might give us endogenous cardioprotective mechanisms activated in response to myocardial ischemia caused by atherosclerotic process. Objective: We aimed to evaluate the clinical profile of non-IRA CTO with regard to cardiovascular risk factors. Methods: We reviewed our invasive cardiology database searching for the non-IRA CTO of any major coronary arteries, and assessed whether or...

Background: The clinical significance of myocardial bridge (MB) on the whole coronary system and coronary atherosclerosis is still a matter of debate. This study aimed to investigate the possible association between MB characteristics (length, depth, proximal stenosis presence and site) with coronary atherosclerotic markers (coronary artery calcification [CAC], plaque, stenosis and pericardial fat volume [PFV]). Patients and methods: From a total of 225 consecutive patients with suspected coronary artery disease who underwent 64-slice multi-detector CT (MDCT) angiography examination, 41 patients with MB were enrolled in the study. Results: MB occurred...

Coronary artery disease is becoming more frequent in young adults with specificities of its own. It is considered as a singular entity, and yet not exceptional one. Methods: We conducted a retrospective study in the Cardiology Depratment B of ibn Sina Hospital in Moroc- co, Rabat between January 2011 and December 2018. The study included all patients admitted with a diagnosis of myocardial infarction and whose age was equal or less than 40 years. The statistical analysis was performed using SPSS Software. Results: In the study period, 80 patients were included. We separated patients in 2 groups: group I with obstructive coronary artery disease (49...

Background: Chest pain is one of the commonest reasons for emergency department visits. Acute myocardial infarction is diagnosed in minority of these patients. Diagnostic algorithms based on high-sensitivity troponin assays enable fast and safe AMI diagnostics. The aim of the present study is to explore effectivity of one-hour (0/1-h) high sensitivity troponin algorithm in management of patients with chest pain in outpatients department of Cardiology department and to describe subgroups of patients. Methods: Patients with chest pain presenting to outpatients department of Cardiology department of Pardubice District Hospital were prospectively examined...

Diabetes mellitus type 2 is a metabolic disease increasing cardiovascular risk. At the same time, the patients with type 2 diabetes are at a greater risk of cardiovascular events, heart failure and chronic renal disease. This review summarizes clinical trials from UKPDS, DCCT and ACCORD till the most recent trials for the new antidiabetics focused on a cardiovascular safety - DPP4 inhibitors, SGLT2 inhibitors and GLP-1 analogues. The overview discusses the particular cardiovascular outcomes, comorbidities and potential renal benefits from the treatment. All new antidiabetic agents were non-inferior with respect to primary cardiovascular outcome, thus...

For decades, anticoagulation therapy has been a cornerstone of comprehensive management of patients with atrial fibrillation. Chronic renal disease is a common condition in a significant proportion of these patients and increases largely the risk of stroke or systemic embolism. Despite clear benefit of anticoagulation therapy in prevention of these complications, the use of this treatment in patients with seriously reduced renal function is still controversial. Although there is a large body of evidence form clinical trials supporting the treatment with novel oral anticoagulants (NOAC) over warfarin in management of patients with mild or moderate reduction...
(In what extent we reached the standards of antiplatelet therapy in secondary prevention of myocardial infarction)

Appropriate pharmacologic control of platelet aggregation represents pivotal treatment target in acute phase of myocardial infarction and in secondary prevention. It is evident for several years, that clopidogrel-based dual antiplatelet treatment does not have a suitable efficacy in several patients. Because of more favorable pharmacodynamics properties, ticagrelor represents more appropriate solution, but its extensive use was opposed by an economical reason. Moreover, recent findings show that patients with impaired glomerular filtration benefit from ticagrelor based treatment in a larger extent than those with maintained normal renal functions....

Authors submit their own experiences with peroral using of amiodarone by patients who are suffering from an atrial fibrillation. They present a brief overview of metabolism of amiodarone, its registered dosage and side effects. By using very low doses and careful monitoring amiodarone is considered as a safe and effective antiarrhythmic.

Atrial fibrillation is the most common rhythm disorder worldwide. Oral anticoagulants are used in the treat- ment of atrial fibrillation. Non-vitamin K oral anticoagulants have been used more often because of the fact that they are more effective and safer than conventional warfarin therapy. The absence of antidote in the majority of new generation oral anticoagulants leads to some difficulties in clinical usage. Idarucizumab, a recently introduced drug, is used in reversing the dabigatran effect in cases of lethal bleeding or acute surgery and invasive procedures. However, little data are available on the efficacy and safety of idarucizumab in geriatric...
Giant cardiac hydatid cyst mimicking coronary artery disease: imaging assessment. A report of two cases

Cardiac hydatid cyst is an extremely rare and uncommon localization of the hydatid disease with deadly complications. We report 2 cases admitted for chest pain and abnormal ECG due to a giant cardiac cystic lesion implanted in the left ventricle. Even though echocardiography is a very sensitive and accessible diagnostic tool, we deepened the investigation using all different imaging modalities for a better approach of the morphological features and specificities of each technique and narrowing the differential diagnosis. Only few cases have been fully illustrated through: TTE, cardiac MRI, contrast-enhanced computed tomography, cardiac computed tomography...
Third degree atrioventricular block as a rare complication of Graves' thyrotoxicosis

Complete atrioventricular (AV) block is a rare but life-threatening complication of thyrotoxicosis. In the literature, only a few cases with uncertain causal link to hyperthyroid status have been described so far. Because of the lack of data, there is no clear consensus on the reversibility of AV conduction abnormality and hence on the need for permanent pacemaker implantation. In the case report, we describe a case of a 35-year-old woman with newly diagnosed Graves' thyrotoxicosis complicated by syncope due to complete AV block. Upon admission to hospital, a spontaneous recovery of the AV conduction with 1 : 1 ratio occurred, resulting in significant...

The most common cause of dementia in younger patients is Alzheimer's disease followed by vascular - most often multi-infarct dementia. Multi-infarct dementia is defined as a cognitive disorder caused by multiple lesions and infarctions in grey and white matter of the brain caused by occlusions of the arteries and arterioles. One of the possible causes of cardioembolic stroke is paradoxical embolism in patients with patent foramen ovale (PFO). We present a case of a 41-year-old man with rapidly progressive dementia with multi-infarct brain damage likely due to paradoxical embolism. He was diagnosed with PFO only after massive pulmonary embolism with...
