Introduction: Obstructive sleep apnea (OSA) is often connected with arterial hypertension and it could also be a cause of secondary hypertension. Treatment of arterial hypertension and optimal blood pressure level are important for prevention of cardiovascular complications. It is not well known how to treat patients with OSA and arterial hypertension. Also many patients with OSA suffer from metabolic syndrome which worsens their prognosis.Aim: The aim of our study was to assess arterial hypertension compensation in patients with metabolic syndrome and moderate to severe OSA and to analyze used pharmacotherapy.Materials and methods: 85 hypertensive...
The determinants of blood pressure response to exercise

Cor Vasa 2015, 57(3):e163-e167 | DOI: 10.1016/j.crvasa.2015.03.010
Introduction: There is currently no consensus on the definition of normal BP (blood pressure) increase during exercise and thus of the exaggerated BP response to exercise. The aim of the present study was a description of the relationship between BP and work rate corrected by body weight on cycle ergometer. A secondary objective was to explore the possible components of this relationship.Materials and methods: An observational study with retrospective analysis of the BP data acquired during standard stress tests on cycle ergometer was performed. For the analysis each work rate was expressed corrected to the patient's body weight. Results: We...

Background: Post cardiac surgery delirium is a severe complication. This study tried to evaluate the early postoperative delirium risk factors and to identify which of them can be modified in order to optimize perioperative management. Methods: It is a prospective observational study. Two hundred and fifty consecutive cardiac surgery patients took part in the study. Cardiac surgery, the anesthetic regiment and the postoperative management were standardized. The incidence and the risk factors of the postoperative delirium were analyzed by univariate and multivariate analysis. Delirium was assessed with screening scale - The Confusion Assessment Method...

Cor Vasa 2015, 57(3):e176-e180 | DOI: 10.1016/j.crvasa.2015.03.007
Introduction: Resection of left atrial appendage (LAA) during surgical ablation of atrial fibrillation may decrease the incidence of postoperative cerebrovascular events and increase the induction and maintenance of sinus rhythm (SR). This study evaluates the effect of LAA resection after the surgical ablation of atrial fibrillation.Materials and methods: A retrospective analysis was made of 599 patients who underwent surgical myocardial revascularization and/or valve surgery with concomitant surgical ablation of atrial fibrillation. The LAA was resected in 140 patients and preserved in 459 patients. Postoperative, 1-month and 1-year results were...
Endovascular management of coarctation of the aorta in adult patients

Cor Vasa 2015, 57(3):e181-e186 | DOI: 10.1016/j.crvasa.2015.03.008
Background: Coarctation is a congenital heart defect with an incidence of 5-8%, more frequent in boys; however, the overwhelming majority of patients undergo surgery earlier as pediatric patients. Currently, the treatment of choice in adolescents and adults is endovascular management.Method: We used all three common catheter-based techniques: simple balloon dilatation, stent placement, and stent-graft placement. All treated patients underwent pre- and postprocedural CT angiography.Group of patients: Between 2004 and 2014, we treated a total of 10 patients with coarctation. They included seven men and three women, with a mean age of 41.1 (21-59)...

Cadmium-zinc-telluride SPECT scanners - New perspectives in nuclear cardiology

Cor Vasa 2015, 57(3):e214-e218 | DOI: 10.1016/j.crvasa.2015.01.001
Nuclear cardiology is one of the most important non-invasive imaging methods in cardiac imaging. It makes possible primarily functional assessment of the heart with quantification of perfusion and systolic function. Development of new types of scanners for nuclear cardiology brings more possibilities in research and clinical practice. This paper describes a brief review of some applications of cadmium-zinc-telluride (CZT) scanners in comparison with conventional cameras.
Medical treatment of hypertrophic cardiomyopathy - What do we know about it today?
Pavel Gregor, Karol Čurila
Cor Vasa 2015, 57(3):e219-e224 | DOI: 10.1016/j.crvasa.2015.02.003
Beta-blockers represent a cornerstone of the treatment of symptomatic hypertrophic cardiomyopathy - they are the first-line therapy for obstructive and non-obstructive forms. Sympathetic modulation leads to the deceleration of heart rate at rest and after exertion, and unconvincingly to the improvement of left ventricular diastolic function and obstruction. Medications include also nondihydropyridine calcium channel blockers without significant vasodilatory properties, i.e. verapamil and diltiazem. Other medications used are class IA antiarrhythmic drugs - disopyramide or cibenzoline, which can be administered to patients with obstructive forms when...

Cor Vasa 2015, 57(3):e225-e227 | DOI: 10.1016/j.crvasa.2014.08.003
We report a case of a successful surgical treatment of congenital heart defect in a 47-year-old man who experienced an atrial septal defect closure seven years ago. Nevertheless even after the operation he still suffered from increasing shortness of breath, which reached grade NYHA III last year. This state was caused by a partial anomalous venous return (PAPVR) from the left lung to the left brachiocephalic vein. This fact was overlooked during the first operation. Surgical treatment of newly diagnosed, at the time undifferentiated concomitant cardiac defect in the field of previous surgery requires a comprehensive assessment of the patient's condition...
Persistent left superior vena cava, absent right superior vena cava and coronary-bronchial fistula: The good, the bad and the ugly (case report and review of literature)

Cor Vasa 2015, 57(3):e228-e233 | DOI: 10.1016/j.crvasa.2014.11.001
A 62-year-old Caucasian male presented with syncope during casual daily activity without preceding prodromes. During ECG Holter monitoring, we observed numerous asystolic pauses lasting >4 s due to sino-atrial blockade and sinus bradycardia. During pacemaker implantation, persistent left superior vena cava with agenesis of the right superior vena cava was diagnosed. Unproblematic placement of atrial lead was followed by challenging placement of the right ventricular lead. Anterior position with a sharp angulation to the right ventricular wall was achieved with excellent stimulation parameters. Transesophageal echocardiography confirmed the diagnosis...
Cardiotoxicity of yew

The Common Yew (Taxus baccata) is an ornamental tree. The taxine alkaloids that are contained in yew berries, needles or bark are poisonous. The lethal dose for an adult is reported to be 50 g of yew needles. Patients who ingest a lethal dose frequently die due to cardiogenic shock, in spite of resuscitation efforts. Although no specific therapy exists, in some patients the asystole phase can be overcome by instituting extracorporeal membrane oxygen therapy (ECMO). Therapeutic procedures reported in the literature are only referred to in published case reports and it is not self-evident whether they have been effective or whether the patient had ingested...

An update of the expert consensus statement of the Czech Hypertension Society on renal denervation in resistant hypertension
Jan Filipovský, Václav Monhart, Jiří Widimský jr.
Cor Vasa 2015, 57(3):e187-e189 | DOI: 10.1016/j.crvasa.2015.01.003
In the spring of 2012, the Czech Society of Cardiology and Czech Hypertension Society issued a joint expert consensus statement on catheter-based renal denervation (RDN) in the Czech Republic. On the basis of new information from the SYMPLICITY HTN-3, PRAGUE-15 and other studies, there is no reason to introduce RDN into clinical practice. New modalities for RDN, such as radiofrequency ablation, are investigated.
