Telemedicine - a discovery of the third millennium?
Ondřej Ošmera, Alan Bulava
Cor Vasa 2010, 52(1-2):55-61 | DOI: 10.33678/cor.2010.011
Telemedicine (telematics for healthcare) provides health care services, activities and systems from one place to another using modern communication technologies. It is a dynamic sphere with a substantial growth in clinical and theoretical applications in chronic disease management. Telemonitoring is the distant evaluation of one's health status. Systems for telemonitoring of patients with implantable cardioverter-defibrillators and cardiac-resynchronisation therapy are broadly used in cardiology. The most advanced is Home Monitoring by BIOTRONIK and it is the only one currently used in the Czech Republic. A number of studies have proved its safety...
Cardiac device-related infection - a new epidemic?
Jitka Vlašínová
Cor Vasa 2010, 52(1-2):62-66 | DOI: 10.33678/cor.2010.012
Despite recent major advances in medicine we are now faced with a relatively new group of patients with cardiac device-related infection. It is necessary to pay close attention to the increasing number of these patients, to identification of the risk factors which predispose to this complication as well as to their specific identification treatment which has its specifics. The aim of this article is to offer an overview of problems experienced by patients with cardiac device-related infection.

Is catheter ablation for atrial fibrillation superior to pharmacotherapy?
Alan Bulava
Cor Vasa 2010, 52(1-2):9-16 | DOI: 10.33678/cor.2010.003
Atrial fibrillation is the most common supraventricular arrhythmia. The incidence of atrial fibrillation in overall population reaches 5% according to some recent estimates. Based on these data, atrial fibrillation together with congestive heart failure is considered to be one of the 21st century epidemics. The presence of this arrhythmia doubles overall mortality and, due to predisposition to thrombus formation, it is responsible for more than one third of embolisation episodes, with a non-negligible socioeconomic impact. Antiarrhythmic therapy for atrial fibrillation is a most challenging issue. Novel non-pharmacological technologies for arrhythmia...
Catheter ablation of ventricular tachycardias: review of literature and own experience
Petr Peichl, Josef Kautzner
Cor Vasa 2010, 52(1-2):17-24 | DOI: 10.33678/cor.2010.004
Catheter ablation of ventricular tachyarrhythmias (VTs) has undergone remarkable development during the last decade. In the majority of patients with idiopathic VTs, catheter ablation represents curative therapy without the necessity of any additional pharmacotherapy. In contrast, in patients with severe systolic dysfunction or rapid VTs, implantation of a cardioverter-defibrillator remains the treatment of choice due to the high risk of sudden cardiac death. Catheter ablation is then indicated in those with recurrent arrhythmias and device discharges. In case of arrhythmic storm, the ablation may represent life-saving therapy.

Catheter ablation of atrial fibrillation (AF) is associated with many technical challenges such as difficult orientation in the left atrium due to significant variability in the arrangement of pulmonary veins, problems with contact between the tip of the ablation catheter and tissue, and increased radiation dose. Two remote control systems have been introduced into clinical practice to overcome these challenges. One of them - an electromechanical navigation system - has already been used in many centres as a standard method. Current experience suggests that this method has many advantages. It allows a standardized ablation procedure, limits fatigue...
Infection of the aorta and great arteries by Salmonella enteritidis

While, in some rare cases, aneurysms of the abdominal aorta or another great artery may be caused by an infectious agent, in other cases, secondary infection may develop in a preexisting aneurysm or even in a non-dilated artery (most often, the aorta or a branch thereof). These cases are invariably very serious and life-threatening. A typical infectious agent with particular affinity to the abdominal aorta is Salmonella enteritidis. The authors present three case reports with the aorta or another great artery infected by this particular Gram-negative bacterium. They discuss the pitfalls of these rare conditions, their diagnosis and, most importantly,...

Primary prevention of sudden cardiac death - ICDs for every patient with ventricular dysfunction?
Lubomír Křivan
Cor Vasa 2010, 52(1-2):30-35 | DOI: 10.33678/cor.2010.006
Despite advances in the diagnosis and treatment of cardiac diseases, sudden cardiac death remains a most topical problem. Implantable cardioverter-defibrillators (ICDs) have been used in sudden cardiac death prevention for almost three decades. A major breakthrough has occurred over the last ten years. The number of secondary prevention ICD implantations has remained nearly stable whereas that of primary prevention ICD recipients has increased. As the number of ICD-indicated patients increases, ICD efficacy declines and costs rise dramatically. The objective of this paper is to put the guidelines for primary prevention ICD implantation into context...

Aim: The aim of this original contribution was to compare the number of patients scheduled for primary preventive ICD implantation according to the old (2005) and new (2009) Guidelines of the Czech Society of Cardiology in patients after ST-segment elevation myocardial infarction (STEMI).Methods and results: We examined 205 consecutive patients 12-29 months after STEMI. We took their history, and ECHO and ECG were performed. No electrophysiology study was performed. Based on these examinations, we assessed the indication for ICD implantation in the primary prevention of sudden cardiac death according to the old and new guidelines. A total of 310...

Aim: Catecholaminergic polymorphic ventricular tachycardia (CPVT) is a rare hereditary arrhythmia. The onset of clinical symptoms usually occurs during childhood, and is typically related to stress. The aim of our study is to provide clinical characteristics of three Czech families with CPVT and pilot results of mutation analysis of the RyR2 gene. Methods: The subjects and their relatives are investigated at the participating departments. They undergo basic clinical investigation, with their history-taking focused on possible CPVT symptoms, i.e. syncopes during stress. Bicycle ergometry is performed to obtain ECG recordings during adrenergic stimulation....

Cor Vasa 2010, 52(1-2):43-48 | DOI: 10.33678/cor.2010.009
Aim: The aims of this prospective study were to define the numbers of responders and non-responders to cardiac resynchronization therapy (CRT) and differences between these groups, and to assess the development of clinical and echocardiographic effects of CRT in the group of responders over a period of time.Group of patients and methods: A total of 58 patients with a biventricular system implanted from July 2005 through May 2008 were included into the follow-up. The mean age of patients was 67 ± 9 years; they were classified in NYHA Class IV (one patient), III-IV (18 patients), III (23 patients), and II (three patients), with average QRS...

Cardiac resynchronization therapy has become an integral part of management of patients with advanced chronic heart failure and cardiac dyssynchrony defined by QRS complex widening in the guidelines. Conceptually, the method is based on mechanical resynchronization of asynchronous, antrioventricular, interventricular, and left intraventricular contractions. A major determinant of resynchronization therapy outcome is the position of individual leads, specifically the left ventricular, right ventricular and, theoretically, also the atrial leads. The review addresses various aspects of identifying the optimal position for individual leads as well as possibilities...
