Objectives: To report on the early outcomes of a prospective single-center registry, which evaluated the feasibility, safety and effectiveness of carotid artery stenting using the FilterWire EZTM Embolic Protection System in high-risk patients. Methods: Between September 2005 and November 2008, 185 consecutive patients (70±9 years, 34% symptomatic, 87% at high risk) underwent 226 carotid artery stenting procedures with the FilterWire EZTM Embolic Protection System and different carotid stents. This study included both symptomatic (≥50% carotid artery stenosis) as well as asymptomatic (≥70% carotid artery stenosis)...

Aim: To demonstrate the complicated evolution of techniques of the surgical treatment of transposition of the great arteries in the world as well as in our country. The successful mastery of the total correction of transposition of the great arteries using our own results has been demonstrated.Evolution of surgery of transposition: Palliative procedures were initiated in the world by Blalock and Hanlon atrioseptectomy in 1948. Successful correction of transposition at the atrial level was started by Senning in 1959 and Mustard in 1963. Correction of transposition at the ventricular level was suggested by Rastelli in 1969, and with correction at...
The characteristics and drug therapy of patients with chronic systolic heart failure of ischemic etiology in the Czech Republic

Introduction: There is a lack of reliable information about the clinical characteristics and treatment of patients with chronic heart failure in the Czech Republic (CR). Nationwide data from the IMPROVEMENT-HF survey carried out in the offices of general practitioners, and the EUROHEART-HF survey in hospitals are inaccurate and have numerous limitations.Aim: To obtain data about the clinical status and drug therapy of patients with chronic systolic heart failure of ischemic origin in the CR.Methods: Data from a total of 497 patients enrolled within the CR into the international multicenter clinical CORONA trial were analyzed. The characteristics...

Background: Enhanced production of reactive oxygen species and decreased antioxidant function play a role in the development of chronic diabetic complications. Objective: To determine whether supplementation with antioxidants (coenzyme Q10 (CoQ10), α-lipoic acid (ALA), α-tocopherol) and simultaneous polarized light (PL) application can improve selected metabolic and echocardiographic parameters of the left ventricle (LV) in diabetics.Patients and methods: Twenty patients with type 2 diabetes were enrolled in a prospective study. Antioxidants were taken in two daily doses (60 mg hydrosoluble CoQ10, 100 mg ALA and 200 mg α-tocopherol)...

In patients with end-stage heart failure, severe pulmonary hypertension is a contraindication to heart transplantation because of the risk for right ventricular failure in the early postoperative period, when the ventricle becomes suddenly exposed to high pressure in the pulmonary circuit. We present the case report of a patient with advanced heart failure and severe pulmonary hypertension, whose -hemodynamic parameters were favorably modulated with sildenafil therapy.
Amyloidosis, a systemic disease with less common involvement of the heart

Amyloidosis is a systemic disease in which amyloid protein is deposited in various organs. One of the clinically significant presentations of the disease is myocardial involvement. In their report, the authors present the case of a man in whom the disease had initially only atypical presentations, yet with rapid progression of problems. It was not until repeat echocardiographic examination was undertaken and combined with the results of other imaging techniques that the correct diagnosis was established.

A challenging area of differential diagnosis in consciousness disorders due to short-term circulatory arrest (syncope) are acute neurological syndromes mimicking syncope. The most common of these syndromes is epilepsy. In about 10% of patients diagnosed to have and treated for epilepsy (mostly those experiencing seizures as the dominant symptom), the underlying cause of a consciousness disorder is short-term circulatory arrest. We report the observation of a patient assessed for recurrent consciousness disorders whose clinical picture of disorders made us consider mainly a neurological cause. The diagnosis - an unusual form of sinus arrest - could...

We report the case of a patient with combined prognostically significant coronary artery disease and chronic renal insufficiency in the presence of renal artery disease, whereby both findings were managed by percutaneous coronary intervention.
