In patients with end-stage heart failure, severe pulmonary hypertension is a contraindication to heart transplantation because of the risk for right ventricular failure in the early postoperative period, when the ventricle becomes suddenly exposed to high pressure in the pulmonary circuit. We present the case report of a patient with advanced heart failure and severe pulmonary hypertension, whose -hemodynamic parameters were favorably modulated with sildenafil therapy.

Amyloidosis is a systemic disease in which amyloid protein is deposited in various organs. One of the clinically significant presentations of the disease is myocardial involvement. In their report, the authors present the case of a man in whom the disease had initially only atypical presentations, yet with rapid progression of problems. It was not until repeat echocardiographic examination was undertaken and combined with the results of other imaging techniques that the correct diagnosis was established.

A challenging area of differential diagnosis in consciousness disorders due to short-term circulatory arrest (syncope) are acute neurological syndromes mimicking syncope. The most common of these syndromes is epilepsy.
