The changing patterns of clinical features and treatment of stable angina in the Czech Republic
Jaromír Hradec, Martina Sachová
Cor Vasa 2007, 49(9):293-301 | DOI: 10.33678/cor.2007.110
Aim:An Angina Treatment Patterns (ATP) survey was performed in 2001 to obtain information about the status of diagnosis and treatment of patients with stable angina in the Czech Republic. A similar survey, ATP III, was conducted in 2006. The results of both surveys were compared. Groups of patients:ATP I enrolled 640 patients while ATP III involved 1,308 outpatients with stable angina. Results:The following changes were documented over the past 5 years, from 2001 to 2006:1. Angina patients were on average 2 years older (69 ± 10 vs. 67 ± 9 years).2. Mean blood pressure levels have increased...
Potential uses of assessment of functional changes in primary hemostasis by the PFA-100 analyzer and modified assessment of platelet aggregation in rationalizing management of patients with familiar hypercholesterolemia treated by extracorporeal LDL-cholesterol elimination

Background:LDL-apheresis is a method for extracorporeal LDL-cholesterol elimination, employed in patients with severe hyperlipidemia refractory to pharmacotherapy and dietary measures. As the method is medically demanding and costly, it is desirable to identify parameters defining optimal intensity and frequency of individual therapeutic sessions. While the tools for long-term evaluation have been identified (imaging methods), appropriate markers of the current status are not yet available. Some studies have suggested normalization of activated platelet function. However, it is not clear to what extent the changes in platelet function are...

Rupture of the free myocardial wall is a rare complication of acute myocardial infarction. Acute rupture is virtually always a fatal complication not allowing therapeutic intervention; even subacute rupture is associated with high mortality. However, its prognosis may be markedly better if diagnosed prior to the development of advanced multiorgan dysfunction or heart arrest.A number of papers have suggested the incidence of free left ventricular wall rupture is lower in patients treated by catheter-based reperfusion therapy. By contrast, the complication occurs more often in the presence of extensive infarction and undeveloped collateral vascular...
Takotsubo cardiomyopathy as an option in the differential diagnosis in transient left ventricular dysfunction

Cor Vasa 2007, 49(9):318-321 | DOI: 10.33678/cor.2007.113
Takotsubo cardiomyopathy is a relatively new clinical entity, which should be considered in the differential diagnosis as a potential cause of complaints in the presence of a negative or insignificant coronary angiographic finding in patients referred for coronary angiography with suspected acute coronary syndrome. The case report presents a 57-year-old female patient with a history of rheumatic fever while young, and a state following subsequent mitral valve replacement for rheumatic heart disease, who was hospitalized in the department of medicine of a catchment hospital for chest pain developing after a negative emotional experience. Because of...

Bradines - a novel approach to the management of myocardial ischemia
Jan Bultas
Cor Vasa 2007, 49(9):323-325
Heart rate is an important factor affecting, in patients with coronary atherosclerosis, perfusion by the left ventricle and the incidence of myocardial ischemia. Heart rate lowering has been used to reduce the ischemic burden since the 1960s with the advent of beta-blockers. An alternative approach resulting in heart rate reduction is inhibition of the specific If ion current in the sinus node. Selective If inhibition by ivabradine, the first representative of a newly formed class referred to as sinus node inhibitors or bradines, is a major addition to the spectrum of drugs available for the management of angina. The mode of action of ivabradine...

Genetics in cardiology. Part IV. Polygenic cardiovascular disease. Potential for gene therapy in cardiology
Ilga Grochová, Ladislav Groch
Cor Vasa 2007, 49(9):327-332 | DOI: 10.33678/cor.2007.115
Part IV of the series discusses the associations between gene polymorphisms and environmental factors in polygenic diseases such as coronary heart disease including myocardial infarction. Despite the hundreds of studies reporting dozens of gene polymorphisms, controversy remains; practical use of genetics including gene therapy in polygenic disease is a matter of future (hopefully not too distant).
Angina, its diagnosis and treatment.
Václav Chaloupka
Cor Vasa 2007, 49(9):334-340 | DOI: 10.33678/cor.2007.116
Angina pectoris is a clinical entity, with its diagnosis based on the patient s self-reported history of pain. This is defined by its location, nature, duration, and relation to exertion. Typical angina is characterized by three basic features: 1. tight chest, 2. it is induced by exertion or stress, 3. it resolves quickly at rest or after the administration of rapid-onset nitrates. Potential or atypical angina presents with only two of these featur and non-specific or anginal pain with only one. Non-invasive diagnosis of angina is based on a thorough assessment of the patient s symptoms and interpretation of the results of diagnostic procedures. The...
