Aim:1. To determine the prognosis of patients treated by primary coronary intervention (PCI) for acute myocardial infarction (MI) requiring mechanical ventilation (MV) in the acute phase of MI.2. To analyze the causes of death of these patients.3. To compare the long-term mortality of the group of acute IM patients receiving MV with the group without MV. Patient group:The long-term prognosis of a group of 510 patients from Prague region, admitted for acute MI (STEMI and non-STEMI) over the period from January 2003 through December 2005, and indicated for PCI, was assessed retrospectively. Results:In...

Left ventricular lateral wall pseudoaneurysm

Left ventricular pseudoaneurysm is a rare complication of myocardial infarction. Since the risk of rupture is many times higher compared with that of true aneurysm, an ability to distinguish one condition from the other is critical for the decision-making on the therapeutic procedure. As symptoms, physical examination, ECG, and chest x-ray are non-specific in most cases, the techniques playing the most important role in the diagnosis of pseudoaneurysm include ventriculography, echocardiography, and more recently, magnetic resonance imaging and cardiac computed tomography. The authors present the case of a 75-year-old female patient developing a lateral...

A rare neurological complication after a cardiac surgical procedure

Cor Vasa 2007, 49(12):460-462 | DOI: 10.33678/cor.2007.160

The authors describe the case of a 61-year-old man with several diseases, admitted to a department of cardiac surgery to undergo elective revascularization for multiple-vessel disease. In the early postoperative period, the patient was shown to have suffered a perioperative myocardial infarction. Massive pharmacotherapy and mechanical cardiac support were required to stabilize the patient s condition. After extubation, the patient s state was again complicated by dysphonia and dysphagia. Given the progression of these conditions, we started an extensive differential diagnosis process (chest x-ray, examination of the ear, nose and neck, gastroscopy,...

The ADVANCE trial published. A fixed perindopril/indapamide combination for all patients with diabetes?

Jiří Widimský

Cor Vasa 2007, 49(12):463-465

The ADVANCE trial is the largest randomized prospective study in type-2 diabetes. The trial compared the effects of addition of a fixed combination of perindopril/indapamide to current treatment versus placebo in over 11,000 patients with diabetes. Active therapy resulted in a significant decrease in macrovascular and microvascular complications as the primary endpoint of the trial. There was also a significant decrease in total and cardiovascular mortality. The total number of coronary events likewise fell significantly. Further, this therapy led to a significant decrease in new-onset microalbuminuria. The main mechanism is thought to be blood pressure...

Mitral stenosis and regurgitation.

Vladimír Rozsíval, Pavel ®áček, Libor Hemľský

Cor Vasa 2007, 49(12):467-480 | DOI: 10.33678/cor.2007.162

This article addresses various aspects of mitral stenosis and regurgitation from communication with the patient, through auscultation to ultrasound examination, and to catheterization. After quantifying mitral valve disease, the pharmacological, catheter-based, and surgical therapeutic options with each condition are discussed.
