Cor Vasa 2004, 45(8):360-364

Introduction:The authors present a summary of hypertension control and treatment in patients on discharge from a department of internal medicine. Group and method:The medical records of patients on their discharge from St Anna's University Hospital Department of Internal Medicine II in Brno were reviewed. There were 2,371 cases of primary hospitalization, with hypertension diagnosed in 59.6% and referred to as the most frequent accompanying diagnosis, followed by coronary heart disease. The mean age of patients was 71 years, and the most frequent form of hypertension was isolated systolic hypertension, present in 34.1%. Discussion:On...

Cor Vasa 2004, 45(8):365-370

Objectives:This study was designed to assess the occurrence of plaque rupture in patients with stable (SAP) and unstable angina pectoris (UAP) by intravascular ultrasound (IVUS). Methods:A total of 107 patients with only non-significant stenoses, as documented by coronary angiography, were included into the study; 62 (57.9%) with SAP and 45 (42.1%) with UAP. The group of UAP patients included those with worsening angina symptoms over the past six weeks or with angina at rest. Results:Overall, 145 ruptures were found in 107 patients (i. e., 1.36 ruptures per patient and artery). Plaque rupture was present in...

Cor Vasa 2004, 45(8):371-378

Aim:The aim of our study was to evaluate the relation of myocardial stress SPECT results to diabetes control, presence of micro-/macroalbuminuria, and echocardiographic results-particularly in relation to the presence of left ventricular hypertrophy and its diastolic function and in relation to carotid ultrasound findings (IMT, intima/media thickness of the common carotid artery, its distensibility and the presence of carotid artery atheroma). Patients and methods:We examined 126 patients with Type-2 diabetes mellitus with two more risk factors according to the guidelines of the American Associations of Cardiology and Diabetes....

Risk stratification of acute pulmonary embolism

Jiří Widimský

Cor Vasa 2004, 45(8):379-383

Mortality of acute pulmonary embolism depends of the hemodynamic stability of patients. Hemodynamically unstable patients with hypotension, or those in cardiogenic shock, show increased mortality rates of 7% and 24.6%, respectively. Stratification of normotensive patients is a more challenging task. The group of normotensive patients at risk can be identified through detection of right ventricular dysfunction whose presence makes the prognosis of patients appreciably worse.Recently, increases in cardiac troponins have been shown to be a marker of right ventricular dysfunction and acute pulmonary embolism severity even in patients free of coronary...

Recurrent left ventricular myxoma

Cor Vasa 2004, 45(8):398-399

Cardiac myxoma is the most frequent benign cardiac tumor, as shown by histology. However, cardiac myxoma is "malignant" to its carrier because of its potential hemodynamic impact and, most importantly, because of the risk for embolism whose consequences may be permanently disabling, or even deadly. The incidence of myxoma is relatively rare, especially so in the presence of a left ventricular myxoma, as is the recurrence of this disease.A total of 63 patients with cardiac myxoma, with only one patient with left ventricular myxoma, were examined in our department over the 1971-1997 period. Recurrence of the tumor was observed in three cases in our...

Insulin resistance and nutrition

Marica Krajčovičová-Kudláčková, Pavel Blažíček, Mária Dušinská

Cor Vasa 2004, 45(8):393-396

Diets rich in carbohydrates with a low glycemic index and a high fiber content are associated with flat postprandial rises in blood glucose, minimal postprandial insulin secretion and maintenance of insulin sensitivity. Protective food commodities in prevention of cardiovascular disease, insulin resistance syndrome or diabetes are crucial components of vegetarian diet. Fasting levels of glucose and insulin as well as calculated values of insulin resistance /IR (HOMA)/ were assessed in two nutritional groups of apparently healthy adult subjects with normal weight (body mass index, 18.6-25.0 kg/m2): a vegetarian group (95 lacto-ovo-vegetarians)...

Cor Vasa 2004, 45(8):384-388

The aim of this communication is to report and analyze the case of a patient with moderate to severe myocardial infarction (left ventricular ejection fraction 28%), treated by primary coronary angioplasty with stent implantation, in whom autologous bone marrow cell transplantation resulted in significant improvement in function, perfusion, and metabolism of myocardial segments sustaining originally irreversible injury. The presence of irreversible injury to the infarct focus was confirmed both by dobutamine stress echocardiography, gated technetium-99m sestamibi SPECT, and positron emission tomography (PET). Cell transplantation was performed on post-infarction...
