Our initial experience with glycogen phosphorylase BB measurement in the diagnosis of acute coronary syndrome

Background:
The commonly used laboratory markers of coronary involvement in subjects with acute coronary syndrome (ACS) are not myocardial ischemia-specific and show late irreversible involvement of the myocardium. A laboratory test has been searched for in order to distinguish individuals with myocardial ischemia and typical CAD symptoms from CAD-free individuals. Since 1989, several papers have appeared on the measurement of glycogen phosphorylase BB (GPBB) with in-house EIA methods describing unique diagnostic sensitivity and specificity of such a test for the assessment of myocardial necrosis and ischemia. Recently, the FDA approved this test for early diagnosis of ACS using standardized ELISA and POCT methods.
Aim and methods:
The present paper summarizes the hypotheses, clinical studies and our preliminary studies in individuals with ACS dealing with this marker.
Results:
Individuals with ACS had higher GPBB values after the admission and 2 hours later. The diagnostic efficacy of this marker is sufficient for ACS presence. Measurement with ELISA and POCT provided comparable results.
Conclusions:
Blood GPBB values had a superior diagnostic efficacy, independent of troponin, within the first hours of the onset of ACS. GPBB POCT measurement is comparable with ELISA test results. Combination of GPBB with troponin could be the optimal approach to the laboratory diagnosis of ACS.
