Cor Vasa 2018, 60(1):e30-e34 | DOI: 10.1016/j.crvasa.2017.12.010
Background: Direct catheter-based thrombectomy (d-CBT) was proven to be an effective treatment for proximal occlusions of the major intracranial arteries in acute stroke patients. The aim of this study was to compare clinical outcomes of patients treated by d-CBT depending on their baseline characteristics. Methods: A single center, prospective, observational registry of consecutive patients (pts) treated by d-CBT for an acute ischemic stroke. The degree of dependence after a stroke was measured by the modified Rankin scale (mRS) at 3 months follow-up and pts were divided into 2 subgroups based on functional independence/dependence (mRS 0-2 vs....

Introduction: Early diagnosis of cardiac allograft vasculopathy (CAV) becomes a crucial step in management of post-transplant patients since it can be attenuated by specific clinical approaches.Materials and methods: We enrolled 48 consecutive patients in this prospective, observational, single centre study. Early development of CAV was assessed by two independent reviewers using quantitative coronary angiography (QCA) in the 1st and 12th month after heart transplantation (HTx). We examined the relationship between CAV and selected clinical and serological variables. Results: A significant mean lumen diameter (MLD) loss was observed in all major...

Atherosclerosis is a progressive, general disease, affecting all arterial beds. Atherosclerosis starts in the youth [1] and aggravates in time, more or less quickly, according to the associated risk factors and genetic background. The simultaneous presence of clinically relevant atherosclerotic lesions in at least two major vascular territories defines the multisite artery disease or panvascular disease (PVD).This represents the situations where ischemic symptoms are present, concomitant, or successive during patient's history and/or in the presence of multiple subclinical arterial lesions at significant risk of clinical manifestation in the...

Cor Vasa 2018, 60(1):e9-e17 | DOI: 10.1016/j.crvasa.2017.12.005
Panvascular disease (PVD) increases significantly the risk for cardiovascular events (myocardial infarction, stroke and cardiovascular death); the more sites affected, the greater the risk of a major cardiac event. Despite its high incidence and severe cardiovascular prognosis, PVD has still not been well studied. History of risk factors and co-morbidities, as well as a detailed physical examination, are mandatory in the initial screening and diagnostic work-up. The ankle-brachial index and various non-invasive imaging methods such as duplex ultrasound, computed tomography or magnetic resonance angiography are used for the diagnosis of atherosclerosis...
Panvascular risk factor - Diabetes

Cor Vasa 2018, 60(1):e18-e29 | DOI: 10.1016/j.crvasa.2018.01.004
Diabetes mellitus is the metabolic bed rock on which macrovascular and microvascular complications develop, hence diabetes is often regarded as a panvascular risk factor. Diabetes, a major risk factor of coronary artery disease (CAD), is also a marker for systemic atherosclerosis. Macrovascular complications include CAD, cerebrovascular disease and peripheral arterial disease (PAD), while microvascular complications include retinopathy, nephropathy and neuropathy. The underlying pathophysiological mechanism of diabetic vasculopathy is complex and multi-factorial, which requires an active research for potential therapeutic targets. The Achilles heel...

Cor Vasa 2018, 60(1):e35-e41 | DOI: 10.1016/j.crvasa.2017.10.001
Polyvascular presentation among patients with peripheral artery disease is common. The cardiovascular burden among those patients highlights the need of minimally invasive therapeutic techniques. Advances in endovascular technology and skills over the last decades offer an alternative to open surgery. This review discusses the cardiovascular morbidity in patients with peripheral artery disease and currently utilized endovascular management.

Cor Vasa 2018, 60(1):e49-e55 | DOI: 10.1016/j.crvasa.2017.10.005
Introduction: Aneurysms develop in up to 60% of patients with an arteriovenous fistula. Frequently arteriovenous fistula aneurysms are asymptomatic with the presence of symptoms potentially heralding the development a significant complication. A range of surgical and endovascular techniques are available to manage arteriovenous fistula aneurysms but clinical guidelines regarding the appropriate application of each approach are lacking. This review will examine the presentation, indications for treatment and management options for arteriovenous fistula aneurysms. Methods: A non-systematic review of published literature in the following databases...
The use of echocardiography in acute cardiovascular care. Summary of the document prepared by the Czech Society of Cardiology

Cor Vasa 2018, 60(1):e70-e88 | DOI: 10.1016/j.crvasa.2017.11.007
Echocardiography is one of the most powerful diagnostic and monitoring tools available to the modern emergency/critical care practitioner, and the provision of echocardiography is fundamental to the management of patients with acute cardiovascular disease. Echocardiography can provide important information throughout the whole patient pathway, having been shown to change therapy in 60-80% of patients in the pre-hospital setting, improve diagnostic accuracy and efficiency in the emergency room, reveal the aetiology of unexplained hypotension in 48% of medical intensive care patients and provide information additional to that obtained from the pulmonary...
Opioids for the treatment of dyspnoea in patients with chronic heart failure

Cor Vasa 2018, 60(1):e89-e93 | DOI: 10.1016/j.crvasa.2017.05.003
Dyspnoea is one of the main symptoms found in patients with chronic heart failure. The awareness of the fact that palliative symptomatic treatment is an important part of the comprehensive approach in the case of the gradual progression of a chronic disease has improved recently. Opioids seem to be suitable medicine for the symptomatic treatment of refractory dyspnoea in patients with advanced chronic heart failure. Randomized controlled clinical trials have proven that the administration of systemic opioids results in the attenuation of dyspnoea. The described mechanisms of the effects of opioids include a reduction in respiratory drive, a reduction...

Cor Vasa 2018, 60(1):e56-e58 | DOI: 10.1016/j.crvasa.2017.04.004
Transcatheter aortic valve implantation (TAVI) is an established treatment option in patients with symptomatic severe aortic stenosis, who are at high or intermediate risk for surgical aortic valve replacement. The vast majority of TAVI are performed on patients without acute decompensated heart failure. In this case report we present the successful TAVI in a patient with a cardiogenic shock.

We hereby present a case report of ruptured sinus of Valsalva aneurysm into the right atrium in 44-year-old patient with 8-month progressive exertional dyspnoea, occasional palpitations and oedema of lower extremities. A massive left-to-right shunt between the outflow tract of the left ventricle and right atrium was discovered from the echocardiogram examination. The patient was indicated for surgery, during which it was discovered a defect in the area of the non-coronary sinus extending into the right atrium, calcifications in the annulus of the congenitally bicuspid aortic valve, without signs of endocarditis present, abundant tissue was excised...

Cor Vasa 2018, 60(1):e101-e103 | DOI: 10.1016/j.crvasa.2016.09.001
Uremic pericarditis, once a major cause of morbidity and mortality, continues to be a severe complication of end-stage renal disease. Its importance and gravity led to the disease illustration in artificial models. The authors highlight the importance of Pathology Museum specimens, namely "wax models", in the teaching curriculum of medical students and young doctors, by showing the similarity between the heart of a recent fatal case and a wax model from the XIXth century.
