Definite evidence has been established, that coronary patients benefit from appropriate secondary prevention. A series of EUROASPIRE surveys evaluated the level of implementation of coronary heart disease (CHD) prevention Guidelines. We aimed to evaluate the changes in adherence to treatment targets during 16 years and to compare the mortality in Czech patients with manifest CHD.Four independent descriptive surveys were undertaken in 1995/1996, 1999/2000, 2006/2007 and 2012/2013. Consecutive patients less than 71 years of age suffering for acute coronary event and/or revascularization procedure were identified and examined at least 6 months afterwards.The...

Introduction: Many patients with coronary heart disease (CHD) who achieve target low density lipoprotein cholesterol (LDL-C) values still experience vascular events because of a residual vascular risk due to other risk factors, particularly non-LDL-C dyslipidemia, because of non-adherence to non-pharmacological and pharmacological management.Method and aims: We used simple markers and inexpensive screening tools for metabolic disorders associated with insulin resistance and metabolic syndrome identifying subjects at a high cardiovascular (CV) risk - atherogenic dyslipidemia (triglycerides [TG] ≥2.0 mmol/l and high-density lipoprotein cholesterol...

Introduction: The main objectives of cardiovascular prevention are, through lifestyle changes, risk factor management and use of cardioprotective medicines to reduce morbidity and mortality, and to improve quality of life. Objectives: To determine, in patients with coronary heart disease (CHD), whether the treatment goals, as defined by current European guidelines on secondary prevention are implemented in clinical practice. Methods: A total of 650 consecutive patients, men and women aged ≤ 80 years when hospitalized for any of following first or recurrent discharge diagnoses or treatments for CHD were retrospectively identified from hospital...

Introduction: Atherosclerosis is the main cause of mortality in the Czech Republic. In our previous cross-sectional studies, we detected a high prevalence of metabolic cardiovascular risk factors in women before and after menopause and found menopausal transition to be critical period for atherosclerosis acceleration. In the present longitudinal study, we studied changes of main cardiovascular risk factors in women after transition to menopause. Methods: We analyzed data of 195 women who became menopausal and 292 women who stayed in menopause during 6-year period. The cardiovascular risk factors under study were as follows: smoking, body mass index,...

Aim: To assess tobacco use and some characteristics of tobacco users (including electronic cigarette users) relevant to cardiovascular disease in a representative population sample of the city of Brno. Methods: A cross-sectional survey of cardiovascular risk factors was conducted using the methodology of the Czech post-MONICA Study, in the city of Brno, Czech Republic in 2013. This preliminary report of the first 965 randomly selected volunteers (including 512 women) aged 25-64, focuses on tobacco use, its prevalence in different subgroups as well as on the attitudes towards smoke-free policies. Results: This preliminary analysis involves 965...

Cor Vasa 2014, 56(2):e128-e132 | DOI: 10.1016/j.crvasa.2014.01.006
Background: Ezetimibe's mechanism of action, complementary to that of statins, makes it a useful therapeutic option in patients intolerant of lipid-lowering drugs or in those not achieving target lipid levels. Objectives: To evaluate the efficacy and safety of ezetimibe in a longterm follow-up of lipid clinic patients with emphasis on motivation for use and the impact on achievement of target lipid levels. Methods: Two hundred and ninety-five clinic patients who were prescribed and took ezetimibe in the 13-month period following the drug's availability in Canada were identified from our database. Patients' history and laboratory data were collected...

Cor Vasa 2014, 56(2):e133-e139 | DOI: 10.1016/j.crvasa.2014.01.003
Purpose: To evaluate the safety and efficacy of combined lipid-modifying agents (statin + fenofibrate) on plasma lipid profile including the atherogenic index of plasma (AIP = log[TG/HDL-C]) in patients at high and very high cardiovascular (CV) risk and mixed dyslipidemia.Method: A total of 81 patients (53 males, 28 females; 60 ± 9.8 years) were included. Mixed dyslipidemia was defined as having 2 of the following 3 lipid abnormalities: LDL-cholesterol (C) >2.5 mmol/l, HDL-C

Cor Vasa 2014, 56(2):e140-e144 | DOI: 10.1016/j.crvasa.2013.12.004
Objective: Our previous 6-month, randomized study demonstrated the beneficial effect of a vegetarian (V) compared to a conventional diet (C) with similar caloric restriction on cardiovascular risk factors for patients with type 2 diabetes (T2D), namely increased insulin sensitivity, reduced body weight, reduced volume of visceral and subcutaneous fat, decreased LDL-cholesterol and improved oxidative stress markers and chosen adipokines. We conducted post-trial monitoring to determine whether the improved outcomes persisted 1 year after the end of the study. Methods: 62 subjects with T2D who completed the study were asked to come for a 1-year follow-up...

Cor Vasa 2014, 56(2):e145-e152 | DOI: 10.1016/j.crvasa.2014.02.010
Introduction: Systemic lupus erythematosus (SLE) is an autoimmune disorder characterized by multiorgan impairment. Cardiovascular involvement represents the leading cause of mortality in SLE patients. Its most common manifestations include pericarditis, valvular affections, conduction disorders, and arterial hypertension. Pulmonary hypertension and coronary arteritis are seen less often. Venous thrombosis directly related to SLE affects about 10% of SLE cases. Acceleration of atherosclerosis is very important and so are the ensuing cardiocerebral events, the most common of those being myocardial infarctions (MIs), cerebrovascular events, thromboembolic...

Introduction: Obstructive sleep apnea (OSA) is considered as a risk factor for the development and worsening of compensation of arterial hypertension and other cardiovascular diseases. Prevalence of masked and nocturnal hypertension can have a significant negative impact on these patients and these prevalences are not well known.Aim: To evaluate the prevalence of masked and nocturnal hypertension in patients with OSA.Materials and methods: In this study, 97 (88 men) patients were enrolled, average age 53.9 ± 9.7 years. OSA was diagnosed with polysomnography and the continuous positive airway pressure therapy has been indicated according...

Cor Vasa 2014, 56(2):e158-e163 | DOI: 10.1016/j.crvasa.2014.02.011
Background: Home blood pressure monitoring (HBPM) is recommended for hypertensive patients as a tool to improve both blood pressure (BP) control and compliance with treatment. Methods: We therefore evaluated the use of HBPM in hypertensive subjects examined during a cross-sectional general population survey (Czech post-MONICA). Models predicting the availability and use of HBPM were constructed using univariate and multivariate logistic regression. Results: Out of 449 treated hypertensive patients (mean age 63.2 years, 52.1% women), 250 (55.7%) subjects reported they had a device for HBPM available at home. Factors associated with HBPM availability...

Twenty years of cardiovascular risk prevention in Czech children

Cor Vasa 2014, 56(2):e164-e168 | DOI: 10.1016/j.crvasa.2014.01.005
Coronary heart disease is the leading cause of death in the Czech Republic. Pathological data have shown that atherosclerosis begins in childhood and that the extent of atherosclerosis in children can be correlated with the presence of cardiovascular risk factors such as: hypercholesterolemia, obesity, hypertension and cigarette smoking. Pediatric preventive program in the Czech Republic, which started in 1992, identifies children at high risk of atherosclerosis. At the age of 5 and 13 years, pediatricians are obliged to measure not only blood pressure and BMI, but also lipid profile (plasma total cholesterol, LDL, HDL cholesterol and triglycerides)...
