Aim: Percutaneous coronary intervention (PCI) became the standard of care for patients (pts) with acute coronary syndromes (ACS). The Czech Republic is among European countries with well developed networks of PCI and non-PCI hospitals. Ample data about PCI-treated pts is available from many registries. Much less is known about treatments and outcomes of ACS pts admitted to hospitals without cath-lab. ALERT-CZ registry was designed specifically to analyze these patients presenting to local non-PCI hospitals. The aim was to see, whether the ESC guidelines are implemented in these local, small hospitals. Methods and results: A total of 6265 pts with...

Objective: The prognostic value of MAC severity is important not only because it shares common risk factors with vascular atherosclerotic changes but also through increasing the cardiovascular morbidity and mortality. This study was conducted in order to evaluate association between severity of MAC and left sided echocardiographic abnormalities mainly left sided valvular abnormalities as well as left atrial systolic and diastolic functional abnormalities. Methods: We prospectively obtained 12-lead electrocardiograms and transthoracic echocardiograms (TTE) on patients scheduled for non-emergent echocardiographic assessment at tertiary care hospital....

Homocysteine is an intermediary product of methionine metabolism. The level of homocysteine is controlled by two pathways - remethylation and transsulphuration. Elevated homocysteine level may result from deficiency or impaired function of enzymes and cofactors in these pathways. Homocystinuria is a rare genetic disease with extreme hyperhomocysteinemia and is associated with the occurrence of arterial and venous thrombotic events at young age. Therefore, homocysteine has been considered as a risk factor for vascular diseases.Plasma homocysteine level is influenced by many factors, genetic as well as environmental. Mild hyperhomocysteinemia is...

Coronary artery ectasia (CAE) is defined as localized or diffuse dilation of coronary artery lumen exceeding the largest diameter of an adjacent normal vessel more than 1.5 fold. The incidence of CAE is reported as 0.3-4.9% of patients undergoing coronary angiography. The rate of recognition may increase with the use of new non-invasive imaging methods as computed tomography (CT) and magnetic resonance (MR) coronary angiography. Atherosclerosis is considered as the main etiologic factor responsible for more than 50% of cases in adults while Kawasaki disease is the most common cause in children or young adults. Coronary ectasia is thought to be a result...

The Takotsubo syndrome is a clinical entity consisting of transient, regional left ventricular (LV) contraction abnormalities in the absence of significant epicardial coronary artery disease. Patients usually present with acute hemodynamic deterioration following an emotional or physical stress. The Takotsubo syndrome is uncommon among patients with end-stage renal disease (ESRD) and patients with congenital coronary abnormalities, such as right coronary artery (RCA) originating from left sinus of Valsalva. Here we describe a patient presenting with acute respiratory distress and anterolateral ST-T segment changes, with negative troponin-I and elevated...
