Our initial experience with genetic testing of patients with hypertrophic cardiomyopathy: mutations of troponin T and troponin I genes

Introduction:Hypertrophic cardiomyopathy (HCM) is an autosomal hereditary dominant disorder caused by mutations in genes encoding cardiac sarcomeric proteins. Troponin T and troponin I make an important part of the heart muscle sarcomere and mutations in their genes are responsible for developing HCM. Acute pulmonary embolism is a relatively frequent condition with a variable clinical course. The authors discuss in detail the current indications for thrombolytic therapy in patients with this condition and the relevance of risk stratification, which should play the pivotal role in future therapeutic decision-making.

In our paper, we report the case of an atypical acute myocardial infarction in a young man with a rare complication-papillary muscle rupture with hemodynamically significant mitral regurgitation-requiring surgery (coronary artery bypass grafting and mitral valve repair). Soon after hospitalization, the patient developed bilateral bronchopneumonia associated with the adult respiratory distress syndrome requiring mechanical ventilation.

The approach to the patient with cardiac insufficiency has changed markedly over the past decade. Current management is focused not only on modulating symptoms of dyspnea and fatigue but, primarily, on modulating the incidence and progression of heart failure and reducing mortality.
