Aim:To compare the cardiovascular risk in patients with significant coronary artery disease associated with aortic stenosis or sclerosis. Method:To compare the clinical, laboratory and angiographic data of consecutive patients with angiographically significant coronary artery disease associated with aortic stenosis (AS) (mean gradient ≥ 30 mmHg) or non-obstructive aortic sclerosis (mean gradient ≤ 10 mmHg). Results:A total of 127 patients with AS (85 males) and 101 patients with aortic sclerosis (76 males) were included into the study. Patients with AS were older (71 ± 7 vs. 66 ± 7 years;...

Tissue Doppler echocardiography in ischemic heart disease - the diagnostic potential for predicting the ischemic substrate and myocardial viability
Tomáš Skála, Martin Hutyra
Cor Vasa 2008, 50(3):109-116 | DOI: 10.33678/cor.2008.041
Tissue Doppler echocardiography (TDE) is an ultrasound modality enabling relatively accurate visualization and quantification of regional myocardial motion and deformation. The aim of this review is to discuss the possibilities of individual TDE modalities in the diagnosis of myocardial ischemia and viability. The paper systematically reviews the individual systolic and diastolic velocity and deformation parameters of TDE, and their potential clinical use in diagnosing myocardial ischemia and in assessing myocardial viability at rest and during dobutamine stress echocardiography before considering surgical revascularization.

Takayasu s arteritis is a rare, idiopathic, chronic inflammatory disease, involving mainly the aorta and its major branches and pulmonary arteries. It leads to stenosis, occlusion or aneurysmal degeneration of large arteries. The clinical presentation depends on the extent and localization of the artery involved. The disease has two phases, the first is acute with nonspecif-ic symptoms, the second phase is chronic in which symptoms are related to stenotic lesions or vessel occlusion.
The issue of infected false aneurysms in drug addicts

The aim of the paper is to discuss optimal management of infected femoral artery pseudoaneurysm in intravenous drug addicts.Judging by the relevant literature, there is no doubt that the most experienced vascular surgeons come from the USA and Iran. Still, there has not been unanimity as to the best option for managing lower limb ischemia as a one-stage procedure.Since December 2006, we have treated three such cases in an attempt to save the patient s life and his/her extremity. The socioeconomic aspects of the issue should also not be disregarded. It seems appropriate to handle each case on an individual basis and to carefully consider...

Indications for heart transplantation

Heart transplantation provides a solution to end-stage heart failure when all other therapeutic options have failed. A successful outcome can only be expected in a properly indicated patient; the decision requires a comprehensive consideration of a variety of medical and social factors. The algorithm of examination includes the assessment of: 1. current status and its reversibility; 2. likely prognosis of the disease while receiving conservative therapy; 3. associated conditions which might represent a contraindication to transplantation; 4. other therapeutic options in heart failure, which might serve as an alternative or a "bridge" to heart transplantation;...

Over the past two decades, orthotopic heart transplantation (OHTx) has become the established method for the treatment of severe heart failure. Challenges faced in the post-transplant period include immunosuppressive therapy and drug interactions, side effects of immunosuppression, rejection, and infection. Main immunosuppressives include calcineurin inhibitors, corticosteroids, antiproliferative agents and, as a separate class, drugs for induction therapy. Side effects of immunosuppression include hyperlipidemia, hypertension, nephropathy, diabetes mellitus, liver dysfunction, osteoporosis, impaired hematopoiesis, and an increased incidence of malignancies....
