Aim:To assess utilization of primary reperfusion therapy in patients with acute phase of ST-segment elevation myocardial infarction (STEMI) in the Czech Republic. Methods:A total of 744 consecutive patients with STEMI from years 2003-2006 were included in the study. Also, data on their demographics and reperfusion strategy were collected. An assessed clinical outcome was in-hospital mortality. Results:The mean age of STEMI patients was 66.9 ± 12.6 years. The vast majority were men (62.4%). A total of 68.6% STEMI patients received primary reperfusion treatment, 97.0% of them were referred for PPCI and...

Aims:The aim of the study was to characterize left atrial tachycardia (LAT) after previous catheter or surgical ablation of atrial fibrillation (AF), and to present results of catheter ablation of LAT. Methods:Ablation of LAT was performed in 40 patients (11 females, 58 ± 10 years) in 42 procedures. Thirteen, 3, and 22 patients underwent previous ablation for paroxysmal, persistent, and permanent AF, respectively, and 2 patients were after surgical cryo-ablation. Stable LAT was mapped and ablated selectively, unmappable alternating multiple LATs were targeted using a substrate approach. The end point of the procedure...

Introduction:Preoperative left ventricular ejection fraction (LVEF) is the most important prognostic factor for surgery for mitral regurgitation (MR). The indications for surgery in patients with MR and severe LV dysfunction remain controversial. The aim of our study was to evaluate the short- and mid-term results of surgery for MR with severe LV dysfunction (defined as LVEF ≤ 30%). Material and methods:From January 2002 through March 2006, we performed mitral valve surgery in 601 consecutive patients. Based on retrospective analysis, we identified 40 patients (30 men, 10 women) aged 65.7 ± 9.4 years, who fulfilled...

Echocardiography is a method used routinely also in the diagnosis of intracardiac formations. Two-dimensional visualization of fast-moving pathological structures within cardiac chambers is fraught with a variety of specific problems and limitations. Their visualization, not always ideal during standard ultrasound examination, still poses a challenge, as an erroneous conclusion and a misdiagnosis may have serious implications for the patient. In theory, three-dimensional real-time echocardiography could mitigate or even completely eliminate some of these shortcomings. This would result not only in shortening procedural time but, also, in improving...

The role of multidetector CT in clinical cardiology.

Cor Vasa 2008, 50(2):83-85
The latest generation of CT scanners employed in clinical practice allows for clinically relevant assessment of coronary atherosclerosis, cardiac morphology and function, including the assessment of some types of valve disease, and aortic stenosis in particular. Currently, the most common indication for assessment using "dual-source CT", or 64-detector CT, in clinical practice is suspected coronary artery disease in low-risk patients. As the body of experience with evaluating the findings grows and the quality of CT scanners improves, the indications are likely to expand rapidly.

The epidemiology of cardiovascular disease (CVD) has been largely instrumental in identifying the etiology of the disease both at the population and individual levels. The knowledge obtained using methods of epidemiological survey has been used to develop a strategy for preventing coronary heart disease (CHD) by modulating risk factors (RFs). This approach to prevention has turned out to be effective in practice. The reductions in mortality, incidence and prevalence of the disease in some countries are largely due to lifestyle modification, with subsequent changes in the pattern of RF distribution in the given population. By contrast, poor lifestyle...
