Our experience with a systematic program of care of adults with congenital heart disease

Aim:A retrospective evaluation of the outcomes of a program of systematic care of adults with congenital heart disease (CHD) at Na Homolce Hospital. Patients:Out of more than 1,900 adults with congenital heart disease, 128 were operated on from April 2005 through January 2007. The mean age was 41 ± 15 years. Results:Surgical mortality was zero as was 30-day mortality; 3 patients were lost to long-term follow-up. The average hospitalization time was 11.7 ± 5 days. Aim of study:Hypertension is often associated with overweight and obesity. Our study was designed to determine the prevalence of hypertension in 5-, 13-, and 18-year-olds and the association of hypertension with overweight and obesity. Method:Anthropometric data were obtained from, and blood pressure measured in a total of 7,427 children attending preventive examinations for children and adolescents at ages 5, 13, and 18 years in general practitioner offices. Malabsorption syndrome as a cause of cardiogenic shock.

This is the case report of a female patient whose initial symptom was upper limb paraparesis, with subsequent development of a shock state, ECG-documented extensive myocardial infarction with acute significant left ventricular dysfunction, all this secondary to malabsorption syndrome in the presence of gluten enteropathy.

The authors report the first cases in Slovakia of ECG-documented Osborne wave associated with hypothermia. They present a review of Osborne wave definitions in the literature and discuss its clinical significance in various clinical settings. They explain the cellular and ionic mechanisms underlying the development of J-wave elevation, and the electrophysiological, ECG, and cardiovascular changes caused by hypothermia.

The authors present the case of a 43-year-old woman with a double-chambered right ventricle. The patient had been observed since youth for a ventricular septal defect that closed spontaneously. At the same time, she gradually developed a right ventricular outflow tract obstruction with a high-pressure gradient. She was scheduled for cardiac surgery for severe symptoms and severe subvalvular pulmonary stenosis.

Arterial hypertension affects 30-70% patients with aortic stenosis. There are no specific recommendations for drug treatment of this population of patients; to date, no consensus has been reached over vasodilator administration. The aim of our study was to determine the incidence of hypertension and the proportion of medically treated patients with aortic stenosis at the time of indication for surgery.Data from a comprehensive examination of 225 patients (mean age 68 ± 9 years; 41% of women) with significant non-rheumatic calcified aortic stenosis (mean gradient 54 ± 17 mmHg) were analyzed. Genetics in cardiology. Part I. The history and evolution of modern genetics

As a specialty, current medical genetics began to evolve in the 1940s to 1950s. The paper provides an overview of the milestones in genetics: discovery of DNA as the carrier of genetic information, uncovering of the DNA structure, protein synthesis encoding, complete identification of genes in the human genome, the HapMap project, ENCODE program, GEI project, and GAIN project. In the second part of the paper, the authors explain the role played currently by genetics in the diagnosis, treatment and prognosis of cardiac disease. The topics of future articles addressing medical genetics are given.
