Introduction: Catheter-based endovenous laser ablation (EVLA) is a widely used alternative to surgery and is slowly becoming a state of the art method in the treatment of varicose veins. In this observational study we sought to determine whether the method is safe and feasible in an unselected population of patients and if it can be used on more vein segments during one procedure. Methods and results: From February 2010 to March 2016 EVLA was performed in 1209 venous segments in 1117 consecutive patients (74% women) with venous reflux. Mean diameter of ablated segments was 9.3 mm (range 5-25 mm). All procedures were performed on an out-patient basis...

Introduction: Cardiac resynchronization therapy (CRT) is currently considered an effective and safe way to treat patients with severe heart failure. Unsuccessful attempts at endovascular insertion of the left ventricular (LV) electrode should prompt a consideration of having the electrode implanted by a cardiac surgeon.Aims: The aims of our work were to evaluate the electric parameters of CS-implanted LV electrodes and to compare them with transvenous implanted electrodes, identify the causes of endovascular implantation failure, compare the clinical and echocardiographic resynchronization effects, and compare the safety of both approaches.Methods...

Cor Vasa 2017, 59(6):e540-e545 | DOI: 10.1016/j.crvasa.2016.12.011
Aim: To assess the left ventricular diastolic function in patients with non-alcoholic liver cirrhosis and correlate the degree of diastolic dysfunction to the severity of liver impairment. Methods: Thirty-five patients with non-alcoholic liver cirrhosis in addition to 16 age- and sex-matched healthy controls were studied. Severity of liver impairment was assessed using the Child-Pugh score. All participants were subjected to echocardiographic assessment using both the conventional and tissue Doppler echocardiography. The left ventricular filling pressure was derived from the transmitral and mitral annular velocities. Results: Patients with non-alcoholic...

Public stroke education: Current status worldwide and projects to increase awareness in the Czech Republic

Cor Vasa 2017, 59(6):e546-e552 | DOI: 10.1016/j.crvasa.2016.12.004
Background: Immediate response to stroke symptoms by a stroke victim or bystander is the first and most critical step for short onset to treatment time, and thus for effective acute stroke treatments. Different campaigns to increase the stroke awareness have been implemented worldwide, but their impact is unclear. Methods: Two databases were explored for papers evaluating stroke educational campaigns. Later, three more papers evaluating campaigns targeted to children were added. All of these papers were evaluated to gain a complex picture and context to introduce Czech educational program HOBIT.Results: Thirty-nine studies were involved into...
HIV as a risk factor of coronary artery disease and of acute coronary syndrome

The number of HIV positive patients in the developed countries is ever growing. Apart from the obvious implications of HIV such as AIDS development, other potentially lethal complications may arise, one of which is the effect of HIV affliction on the coronary system. HIV patients may develop coronary artery disease at a very young age and the myocardial infarction in these patients can be hard to correctly diagnose at the beginning due to the often confusing clinical picture. This paper summarizes the current knowledge on the association between HIV and coronary artery disease and illustrates it on two case reports.
Pulmonary valvular and pulmonary arterial myxomas
Shi-Min Yuan
Cor Vasa 2017, 59(6):e557-e561 | DOI: 10.1016/j.crvasa.2017.01.008
Background: Cardiac myxomas are rarely arising from the pulmonary valve or pulmonary artery.Materials and methods: An extensive literature search generated 22 articles with 22 patients, which were taken as the material of this comprehensive review. Results: Most patients manifested as circulatory system symptoms. A heart murmur was noted in 17 (94.4%) patients. Pulmonary valve and pulmonary artery myxoma was misdiagnosed in 4 (22.2%) patients. Pulmonary myxoma was misdiagnosed in 5 (22.7%) patients. It was misdiagnosed as pulmonary valve stenosis in 2 (40%), as pulmonary artery embolism in 2 (40%), and as pulmonary valve vegetation or cardiac...
