Cor Vasa 2017, 59(3):e215-e221 | DOI: 10.1016/j.crvasa.2016.12.014

The landmark trials MADIT II, SCD-HeFT, and COMPANION in 2002-2005 years have reported their positive results to sudden cardiac death reduction. Since that time the indications for the use of implantable cardioverter-defibrillators (ICDs) have substantially broadened. The occurrence of appropriate ICD therapy differs in the individual trials. We were retrospectively analyzing the occurrence of ventricular tachycardia/ventricular fibrillation (VT/VF) from ICD remote monitoring database (Biotronik Home Monitoring TM, www.biotronik-homemonitoring.com). No significant difference was found between subgroups divided by the implantation indication, the programmed...

Background: Postoperative atrial fibrillation (POAF) is observed in the early postoperative period in approximately every third patient after coronary artery bypass grafting (CABG). The pathogenesis of POAF is multifactorial and is not yet fully studied. In many studies, post-operative inflammatory response has been extensively investigated as a potential basic factor of POAF. It is known that statins have anti-inflammatory properties. In some studies, pre- and perioperative use of statins has shown the decrease of incidence of POAF after CABG.Objective: We conducted meta-analysis of randomized and observational studies of efficiency of statin...

Cor Vasa 2017, 59(3):e272-e276 | DOI: 10.1016/j.crvasa.2016.12.013

Peripartum cardiomyopathy (PPCM) is rare, but-life threatening disease, which affects women in the last month of pregnancy or in the first five months after delivery. Its incidence is variable, ranging from 1 in 1300 to 1 in 15,000 pregnancies and is more prevalent in Haiti and certain parts of Africa. Risk factors include multiparity, advanced maternal age, multiple pregnancies, pre-eclampsia, chronic hypertension, smoking, alcoholism, malnutrition, and long-term tocolysis. Our case report describes a case of a patient, who was admitted for shortness of breath rapidly worsening into asthma cardiale closely after spontaneous delivery. As the cause...

Cor Vasa 2017, 59(3):e277-e281 | DOI: 10.1016/j.crvasa.2017.01.004

A 78-year-old woman with a history of ischemic heart disease and permanent atrial fibrillation, on treatment with warfarin, was admitted to the Emergency Department because of paroxysmal nocturnal dyspnoea. Instrumental findings indicated an early phase of acute heart failure. The latest INR value was 3.64, and previous available INR values were all within the therapeutic range (2-3). A transthoracic 2D echocardiographic examination showed left ventricular dilatation and a severe reduction in systolic function. An echogenic pedunculated mass was observed in the left atrium, adherent to the interatrial septum. Twelve hours later, the patient reported...

Unusual cases of acute mitral valve regurgitation

Jiří Lukavec, Hana Línková, Robért Petr, Zbyněk Straka

Cor Vasa 2017, 59(3):e282-e286 | DOI: 10.1016/j.crvasa.2016.07.002

Severe acute mitral regurgitation without early surgical correction can lead to congestive left-sided heart failure and quick death. Traumatic mechanism is one of its rare causes and is described in the literature mainly after blunt chest injuries in car accidents. In our case reports, we deal with cases of patients who have been diagnosed with a significant mitral regurgitation with rupture of chordae tendineae with symptoms of left ventricular failure requiring cardiac surgery. The onset of symptoms was associated with traumatic events of small or mild intensity.

Endovascular treatment of the malignant thrombus in the descending thoracic aorta

Michael Jonák, Ondrej Majchrák, Jiří Novotný, Olga Ternuscsák

Cor Vasa 2017, 59(3):e287-e290 | DOI: 10.1016/j.crvasa.2016.12.003

Thrombus in the non-aneurysmal, non-atherosclerotic descending thoracic aorta (NAADTA) represents a rare source of peripheral arterial embolism. Despite being mostly asymptomatic process, its consequences can be very serious. In this case report, we described the case of a patient with malignant thrombus occurring in otherwise ''healthy'' descending thoracic aorta, already complicated by embolization into superior mesenteric artery, subsequently solved by stent graft implantation into the thoracic aorta.

Infective endocarditis and incidental popliteal artery mycotic aneurysm

Serkan Burç Deşer, Mustafa Kemal Demirağ

Cor Vasa 2017, 59(3):e291-e293 | DOI: 10.1016/j.crvasa.2016.12.012

A mycotic aneurysm (MA) is rarely seen and defined as a pseudoaneurysm of the vessel wall due to infective endocarditis (IE). Arterial complications are often seen as an acute ischemia in the limbs (20-30%) and usually involve the lower limbs rather than upper limbs. Majority of MA involve brachial, femoral or popliteal arteries with their branches however, MA is rarely seen on the distal part of the popliteal artery. Popliteal artery mycotic pseudoaneurysm is rare and may imitate calf vein thrombosis. We report surgical management of a rare case of MA involving the popliteal artery and aortic valve replacement (AVR) due to IE.
