Background: Percutaneous coronary intervention (PCI) in patients with acute coronary syndrome (ACS) should be performed in presence of objective evidence of myocardial ischemia. Our study investigated the appropriateness of PCI among ACS patients in Russia and explored clinical factors associated with PCI performance. Methods and results: Clinical information about 65,912 ACS patients (60.5% male, aged 63.2 ± 13.8 years) enrolled in the 2010-2011 Russian ACS Registry was examined. ACCF 2012 criteria were used to assess the appropriateness of PCI. PCI was performed in 13.8% of patients included in the study. Among patients with performed PCI...
Distribution of electrical reconnection after pulmonary vein isolation using a duty-cycled radiofrequency circular ablation catheter

Cor Vasa 2014, 56(1):e11-e18 | DOI: 10.1016/j.crvasa.2013.12.002
Background: Circular ablation catheters (PVAC) have been shown to be effective in the treatment of patients with paroxysmal atrial fibrillation (PAF). The electrophysiological characteristics of the pulmonary veins (PVs) during repeat procedures remain unknown. Objective: To assess the efficacy of PV isolation (PVI) using PVAC and to determine the typical sites of electrical reconnection (ER) of PVs.Methods: 79 patients with PAF underwent PVI using PVAC. Those who remained symptomatic underwent repeat procedure using a high-density 3D electroanatomical mapping to determine the sites of ERs.Results: In total, AF recurrence was documented...

Catheter ablation of atrial fibrillation (AF) is a highly effective therapy to achieve freedom of recurrent arrhythmia and relief from symptomatic AF. Transmural ablation of atrial tissue is crucial for success. Thus steerable sheaths and catheter design with contact measurement as additional feature have been developed to increase success rates. New 3 dimentional (3D) mapping technologies engage clinical routine to reduce fluoroscopy time and radiation dose for patients and medical staff to a minimum. To unmask dormant pulmonary vein reconduction and to avoid early pulmonary vein reconduction administration of adenosine is useful. Future approaches...
Congenital coronary anomalies

Background: Congenital coronary anomalies occur rarely and affect 0.3-1% of the population [1,2]. They are predominantly clinically silent and are usually diagnosed accidentally during selective coronarography for other reasons, or at autopsy. In some cases however these maybe the cause of sudden death, mainly in young individuals in association with great exertion (anomalous origin of coronary artery from the opposite coronary sinus with interarterial course).Method: We reviewed the cathlab records and coronary multi-slice CT scans performed in the period from 2008 to 2013 in our hospital and we chose typical image findings documenting particular...
Reaching betablockers target dose in elderly patients with chronic heart failure
Filip Málek
Cor Vasa 2014, 56(1):e37-e41 | DOI: 10.1016/j.crvasa.2014.01.001
Despite overwhelming evidence of clinical efficacy, the use and dose of betablockers in elderly patients with chronic heart failure is often suboptimal. The underuse and underdose of betablockers in elderly may reflect true intolerability in older patients with commorbidities and with increased risk of side effects. Different betablockers may have different side effects because of different pharmacological properties. Difference between betablockers use in the clinical practice and clinical trials might be explained by the fact that the major large-scale betablockers trials enrolled younger patients. The highest tolerability in elderly heart failure...

Coexisting coronary artery disease and significant heart valve disease represent a considerable risk factor in patients undergoing pulmonary resection. The possibility to perform concomitant (simultaneous) heart and lung surgery could be a good option for many patients. In the presented case report, the intervention was performed on two valves, triple coronary artery bypass and pulmonary resection, which has not been published in literature to date.A 68-year-old woman with coronary artery disease, aortic valve stenosis, tricuspid valve regurgitation and pulmonary disease (malignant pathology) presented with triple vessel disease, moderate aortic...
Mediastinal abscess compressing the left atrium: A case report and literature review

Cor Vasa 2014, 56(1):e80-e83 | DOI: 10.1016/j.crvasa.2013.06.008
The left atrium is a thin-walled cardiac chamber with a low intraluminal pressure. It is located in the inferoposterior part of mediastinum, near to structures such as the esophagus and descending aorta. This makes it vulnerable to compression by pathologic changes associated with the gastrointestinal tract, mediastinum, lungs, pericardium and aorta. Depending on the level of the compression the patient may be asymptomatic or develops signs of cardiac insufficiency. We describe a lethal course of left atrial compression due to a chronic mediastinal abscess secondary to a ruptured esophageal ulcer. To our knowledge this is the first such case described,...
