Introduction: Cardiac resynchronization therapy is a therapeutic option in patients with congestive heart failure. Surgical epicardial lead placement for biventricular pacing is the method of choice after failed coronary sinus (CS) cannulation. We describe our initial experience with minimally invasive surgical lead implantation using video-assisted thoracoscopy. Methods: From August 2008 to January 2010, a total of 12 patients (mean age 71.9 ± 8.9 years) with congestive heart failure, NYHA functional class 2.9 ± 0.3 and depressed ejection function (25.5 ± 5.0%) were referred for surgery because of failed CS left ventricular...

Diastolic dysfunction and markers of collagen turnover
Jaroslav Meluzín, Josef Tomandl
Cor Vasa 2010, 52(9):494-499 | DOI: 10.33678/cor.2010.135
The purpose of this paper is to summarize the current knowledge of the relationship of markers of collagen turnover to left ventricular (LV) diastolic dysfunction. There exists a strong relationship between the predominance of collagen synthesis over its degradation, myocardial fibrosis, and LV diastolic function. Some serum markers of collagen turnover were found to be practical surrogates of collagen content within the myocardium and indicators of the severity of LV diastolic dysfunction. However, the selection of optimal individual biomarkers for determination of various pathological clinical situations associated with disturbances in collagen turnover...
Smoking and cardiovascular disease

Smoking is an established risk factor for the development of atherosclerosis; active as well as passive smoking are involved in the development and progression of atherosclerosis as well as its unstable forms, i.e., acute coronary syndrome. As is evident from the Czech Guidelines for Tobacco Addiction Treatment, smoking is the cause of one of five deaths throughout Europe and in the Czech Republic, resulting in an 18,000 deaths annually in the latter. The effect of smoking on the development of cardiovascular disease is mediated by a variety of mechanisms. These include genetic predisposition, altered endothelial vasomotor function, structural endothelial...
PET/CT in cardiology

Cor Vasa 2010, 52(9):506-512 | DOI: 10.33678/cor.2010.137
PET/CT is a recent noninvasive imaging technique which put together two sophisticated techniques into one device. It provides two basic types of information about the structure and function of the human body which complement each other not only on the level of technology but also biology. PET/CT serves partly in clinical practice mainly in the work-up of patients with ischemic heart disease, and partly in experiment, particularly in the area of molecular biology. This article briefly describes the principle of both techniques and explains the complementary role in the evaluation of myocardial perfusion and metabolism. Its potential uses in the near...
Advances in myocardial perfusion and left ventricular function imaging using single photon emission tomography

This article is a review of recent progress in stress myocardial perfusion imaging using single photon emission computed tomography (SPECT). This examination provides effective and cost-effective diagnostic and prognostic information in patients with known or suspected coronary artery disease, in the detection of ischemia in patients after coronary revascularization, and in viability assessment. ECG-gated SPECT examination is a current standard which makes the evaluation of left ventricular myocardial perfusion and function possible. Quantification of the extent and severity of perfusion defects by means of a summed stress and difference scores provides...
Options in reconstructive procedures in patients with a bicuspid aortic valve

The bicuspid aortic valve is the most common congenital heart disorder potentially resulting, among other things, in the development of aortic valve stenosis or regurgitation. In cardiac surgery, there has been an increasing tendency in the last decade toward aortic valve sparing procedures, even in the presence of the bicuspid aortic valve. The benefits of sparing the native aortic valve include elimination of risks associated with replacing the native valve with a prosthetic one. These are thromboembolic and bleeding complications due to permanent anticoagulation treatment in patients with mechanical valves, the risk of premature degeneration of...

Acute ST-elevation myocardial infarction in a young male with rectal adenocarcinoma

Cor Vasa 2010, 52(9):532-533 | DOI: 10.33678/cor.2010.140
The authors present the case of a man developing an acute myocardial infarction when only 35 years old. A subsequent bleeding complication while receiving antiplatelet and anticoagulation therapy led to the finding of squamous cell carcinoma in the rectal region; the carcinoma was successfully removed. Patients developing acute myocardial infarction associated with a bleeding complication require comprehensive assessment including cancer screening tests even in young individuals.

Management of care for healthcare personnel at risk of exposure to ionizing radiation
Jana Golisová, Otakar Kraft
Cor Vasa 2010, 52(9):564-567
Objective: To highlight the importance of care for employees exposed to ionizing radiation as a risk factor and to document radiation levels in selected groups of healthcare personnel.Areas of care for the personnel: Management of care for employees exposed to and at risk of ionizing radiation in medicine is quite a large and challenging area with the potential of modifying the rates of work-related accidents and occupational diseases as well as the health of the personnel and its understanding. Management includes safety and protection of health at work (radiation hygiene), and healthcare provided to the personnel (also above standard) by the...
