The optimal therapeutic strategy for patients with multiple vessel disease can only be defined in close interdisciplinary cooperation and with comprehensive assessment of total and coronary risk. In addition to morphological or anatomical aspects, increasing attention should also be given to a functional point of view, which may be helpful in making treatment more effective and safer. Results of the well-designed FAME and SYNTAX trials may render our decision-making more precise and, conceivably, also easier.

Selective coronary angiography is the gold standard for assessing the coronary arteries. However, some findings are difficult to interpret. This review article aims at summarizing current knowledge regarding complementary invasive methods for assessing the coronary arteries in terms of their hemodynamic function.

The authors offer an overview of the most frequent indications for intravascular ultrasound (IVUS) and virtual histology (VH) in the catheterization laboratory, with an emphasis on routine use of these techniques in coronary interventions. The procedure for diagnosing hemodynamically significant lesions by IVUS in coronary interventions is described. VH is a suitable method, particularly for assessing atherosclerosis progression and for risk stratification of atherosclerotic plaques. However, there is already a body of data available that can be potentially made use of in real life.

Optical coherence tomography - a new tool for coronary artery imaging

The authors concisely discuss the imaging potential of optical coherence tomography as another major advance in imaging individual structures of the vessel wall and assessing the pathological processes that occur there, particularly for the detection and evaluation of atherosclerotic plaques and, subsequently, for assessing the outcome of treatment of coronary arteries.
Acute coronary syndromes without persistent ST elevation in the light of the 2007 and 2008 guidelines

New guidelines for the management of acute coronary syndromes without persistent ST elevation were published in 2007 and 2008. The paper addresses new and several somewhat neglected aspects of these guidelines. The new guidelines highlight the need for preventing bleeding complications and for a balanced management in terms of benefits of ischemia/bleeding. Methods of risk stratification are employed to optimize therapy, and mention is also made of the occasionally neglected risk scores.

Contrast-induced nephropathy

Contrast-induced nephropathy is a relatively common complication associated with the use of iodinated contrast media during cardiac catheterization procedures. Although the increase in serum creatinine is transient in most cases, it is generally associated with increased morbidity and mortality rates, especially in the high-risk population. This review summarizes current findings regarding the pathophysiology and strategy for preventing and treating this complication.
