Aims: Patients with end-stage renal disease have high morbidity and mortality rates. The one-year mortality is 20-23%, with cardiovascular disease accounting for almost 43%. The aim of the present study was to describe cardiovascular epidemiology in patients referred for specialist ultrasound examination of their grafts.Methods: In our department, we investigate patients with grafts every 3 months after access creation. Cardiopulmonary exercise testing (spiroergometry) provides a comprehensive perspective of exercise physiology and pathophysiology. The article summarizes indications and overviews current knowledge and evidence concerning clinical use of the method in practical cardiology. Attention is given to results that can be used to establish the prognosis of and to treatment in patients with heart failure and, in the second part, to the contribution of the method to the differential diagnosis of dyspnea. The mainstay of treatment of pulmonary arterial hypertension is specific pharmacotherapy (with prostanoids, endothelin receptor antagonists, phosphodiesterase 5 inhibitors). Patients with severe pulmonary arterial hypertension have recently been shown to benefit from combination specific therapy as this therapy - by affecting several pathogenic mechanisms - is capable of stabilizing the clinical presentation of this serious condition necessarily deteriorating over time.

ENHANCE trial. Effect of Combination Ezetimibe and High-Dose Simvastatin vs Simvastatin Alone on the Atherosclerosis Process in Patients with Heterozygous Familial Hypercholesterolemia

The paper presents the results of the ENHANCE trial, a double-blind, randomized, 24-month study assessing 720 patients with familiar hypercholesterolemia. The study was designed to compare the efficacy of lipid-lowering therapy with 80 mg simvastatin + 10 mg ezetimibe versus therapy with 80 mg simvastatin on carotid and femoral intima-media thickness (IMT), as assessed by repeat ultrasound. Combination therapy did not result in significant changes in IMT over monotherapy, although simvastatin plus ezetimibe significantly reduced the levels of LDL-cholesterol and C-reactive protein compared with the group of patients treated with simvastatin alone....

In the industrialized nations, a lower socioeconomic status (SES) is associated with higher cardiovascular (CV) morbidity and mortality as a result of a higher prevalence of smoking, overweight and obesity, lower physical activity, higher blood pressure, more frequent hyperlipidemia and diabetes, adverse psychosocial factors (social exclusion, stress, depression, anxiousness, hostility) and poorer patients' compliance with physician's instructions regarding prevention and treatment among individuals of lower social standing.
