Aim of study:A number of experimental and clinical studies have shown that right ventricular (RV) apical pacing leads to marked asynchrony of ventricular activation. This can result in systolic and diastolic left ventricular (LV) dysfunction. A solution is pacing from alternative RV sites. The study was designed to compare the effect of septal vs. apical pacing using echocardiographic parameters of ventricular dyssynchrony on mid-term follow-up. Method:Forty-one patients (31 men, mean age 73 ± 10 years) with advanced AV block and preserved LV ejection fraction (EF > 45%) were included into the study. Patients were...

Introduction:The main objectives of prevention are to reduce morbidity and mortality, and to improve quality of life. Aim:To determine, in patients with coronary heart disease, whether the treatment goals, as defined by current guidelines on secondary prevention, are implemented in clinical practice. Methods:A total of 600 consecutive patients, men and women aged ≤ 80 years, were identified retrospectively with the following diagnoses: coronary artery bypass grafting, percutaneous transluminal coronary angioplasty or acute coronary syndrome. Data collection was based on a review of medical records and interview...

Balloon aortic valvuloplasty - a forgotten method or hope for the high-risk elderly patient?

Today, calcified aortic stenosis is the most common type of heart valve disease whose prevalence increases with increasing age. The gold standard in the treatment of calcified aortic stenosis is surgical aortic valve replacement, which is currently the most common valve-related surgical procedure. Balloon aortic valvuloplasty (BAV) involves dilatation of a stenotic aortic orifice using one or two balloon catheters. According to current European guidelines, those indicated for BAV include patients with symptomatic severe aortic stenosis, as a bridge to surgical aortic valve replacement in hemodynamically unstable patients at high surgical risk, or those...

Hybrid, off-the-pump management of atherosclerotic ulcers of the aortic arch.

Martin Urban, Lubomír Blaha, Marian Wierzgoň, Piotr Branny, Daniel Říha, Stanislav Holesz

Cor Vasa 2008, 50(4):167-169 | DOI: 10.33678/cor.2008.056

In addition to aneurysms, atherosclerotic ulcers of the aortic arch are a clear sign of extensive atherosclerotic lesions involving the aorta and brachiocephalic branches of the arch. The ulcers are the clinical equivalent of aneurysms and pose, among other things, the risk of rupture with bleeding fatal for the patient. Conventional surgical management requires the use of the extracorporeal circuit. The procedure is performed in deep hypothermia with circulatory arrest and selective brain perfusion or, alternatively, in mild hypothermia with selective perfusion of the brain and the lower part of the body. These procedures are associated with higher...

The diagnosis, epidemiology, and prognosis of acute heart failure.

Jindřich Špinar, Lenka Špinarová, Jiří Vítovec

Cor Vasa 2008, 50(4):171-175 | DOI: 10.33678/cor.2008.057

Acute heart failure is a disease whose incidence is comparable with that of acute myocardial infarction. It is estimated about 4,000 persons/million/year are hospitalized with that diagnosis in the Czech Republic. The most common causes are chronic heart failure decompensation (about half of patients) and acute coronary syndrome (one in three patients). The mainstay of diagnosis is clinical examination, complemented with x-ray, electrocardiography, echocardiography or another imaging technique, and biochemistry, with determination of natriuretic peptides being the most valuable diagnostic tool. The prognosis of the disease is poor, with in-hospital...

Managing acute heart failure.

Jindřich Špinar, Ivan Málek

Cor Vasa 2008, 50(4):177-182 | DOI: 10.33678/cor.2008.058

Acute heart failure is a condition associated with high mortality rates and one difficult to treat. As no large mortality studies demonstrating an effect of treatment on survival have been reported, treatment is based on experience and is largely empirical. Factors central to the decision regarding the therapy to be used include the patient s clinical condition and the cause of heart failure. The main class of drugs used in pharmacotherapy are diuretics, with vasodilators used in patients with a hypertensive response while, conversely, positive inotropic agents are administered to those with hypotension. All these modes of action are combined in...
