Our experience with aortic bioprostheses

Objective:Valve replacement surgery in the case of degenerative aortic stenosis is a most frequent procedure. The purpose of this study was to compare the hemodynamic parameters of different types of bioprostheses. Methods:Between March 2002 and September 2006, a total of 277 aortic bioprostheses were implanted at our institution (70% of all aortic prostheses). The bioprostheses used were as follows: Edwards Lifesciences (models 2650, 2900, 3000), Medtronic-Hall (Mosaic), SJM (Epic, Epic supra), and Sorin (More, Soprano). One hundred and fifty four men and 123 women, mean age 74 ± 6 years, were included into the study....

Post-infarction ventricular septal defect is a relatively uncommon condition, generally considered serious because of its high morbidity and mortality rates. The authors describe percutaneous closure of a post-infarction ventricular septal defect in an elderly female patient who had had surgical revascularization because of a subacute myocardial infarction. A significant muscular ventricular septal defect developing postoperatively resulted in hemodynamic instability of the patient.Percutaneous closure of the defect was obtained using an Amplatz occluder. This case report presents the symptomatology, clinical finding, diagnosis, and treatment of a female patient with a left atrial myxoma, highlighting the crucial role played by echocardiography as a widely available and non-invasive screening technique. The introduction provides an overview of currently known facts about the etiopathogenesis, clinical features, and diagnosis of cardiac myxomas.

Genetics in cardiology. Part III. Monogenic inheritance syndromes and cardiac disease

This part of our series addresses monogenic (Mendelian) inheritance disease. The following most frequent monogenic inheritance disease with cardiac symptomatology are discussed: Di George syndrome, Noonan syndrome, Holt-Oram syndrome, Marfan syndrome (including detailed diagnostic criteria), hypertrophic cardiomyopathy, dilating cardiomyopathy, non-compact cardiomyopathy, Fabry s disease, arrhythmogenic right ventricular dysplasia (including the diagnostic criteria), long-QT syndrome and Brugada syndrome. The type of inheritance, the risk for disease transmission to the offspring, and the potential of molecular genetic diagnosis are given with each syndrome.

The main goals of preventive cardiology are:1. to prevent the development of the most frequent cardiovascular (CVD) and/or atherosclerotic vascular (AVD) diseases (primary prevention) and 2. to prevent their progression, recurrence of acute events and related morbidity (secondary prevention). Despite the downward trend of mortality from coronary heart disease, 50% of all acute coronary events continue to kill those affected before professional care can be provided! An epidemic of Type-2 diabetes is expected in this century, resulting in a further rise in the incidence, prevalence, and mortality from AVD. These facts pose an enormous challenge...
