This review article summarizes in detail the indications for coronary artery bypass grafting (CABG) based on clinical criteria, coronary angiography finding, left ventricular function, and operative risk estimate using various techniques. Utmost emphasis is placed on operative risk estimate using what is referred to as the EuroSCORE. The long-term outcomes of CABG are also addressed. The authors clearly identify conditions under which it is preferable to perform surgery or to institute conservative therapy. The differential diagnosis of chest pain is one of the most common problems faced by physicians of emergency medicalservice and outpatient departments of internal medicine and neurology.The paper is the case report of a 31-year-old man with a post-traumatic mediastinal cyst diagnosed at the Department of Medicine of the Brno-based Military Hospital and subsequently removed during scheduled on-pump surgery in the Cardiovascular and Transplant Surgery Center. Postoperative complications are also briefly mentioned.

Managing diastolic heart failure. Results of PEP-CHF

PEP-CHF (Perindopril in Elderly People with Chronic Heart Failure) was a one-year, randomised, double-blind study comparing the effects of the ACE inhibitor perindopril and placebo on mortality and morbitidy in elderly patients with chronic heart failure secondary to diastolic left ventricular dysfunction. This was the first study of chronic heart failure assessing diastolic left ventricular dysfunction by echocardiography. The study included 850 patients with a mean age of 76 years. It included cases of mild to moderate chronic heart failure, as most patients (74-77%) were classified in NYHA Class II/I, as also reflected by low overall mortality rates....
