This brief overview discusses the potential differences in the metabolic action of different classes of antihypertensive agents in relation to the development of diabetes mellitus. A meta-analysis of large studies of antihypertensive therapy has shown renin-angiotensin system inhibitors are associated with the lowest risk of developing new-onset diabetes mellitus. New-onset diabetes mellitus may significantly raise the cardiovascular risk in the long run.
