Plasmatic levels of inflammatory markers during cardiopulmonary bypass in hypothermia and normothermia

Introduction: The aim of our study is to find out if normothermia during cardiopulmonary bypass (CPB) decreases the intensity of systemic inflammatory response in patients compared with mild hypothermia around 30 °C. An important marker of the stress reaction during CPB is cytokine production.
Methods: The plasmatic levels of diferent cytokines were measured eight times before, during, and after cardiac surgery. Two groups of patients - normothermic (n = 23) and hypothermic (n = 20) - undergoing elective coronary revascularisation were compared. We measured and compared the plasmatic levels of the following cytokines: TNF-α, interleukins IL-1-β, IL-6, IL-8, IL-10, endothelin ET-1, intracellular adhesion molecule ICAM-1, vascular cell adhesion molecule VCAM-1 and L-selectin.
Results: We found significantly decreased levels of proinflammatory cytokines (TNF-α, IL-6 and IL-8) and increased level of the antiinflammatory cytokine IL-10 in the normothermic group in comparison with the hypothermic group.
Conclusions: As for the development of a systemic inflammatory response, normothermia appears less harmful in comparison with hypothermia.
