Myocardial ischemia detection using albumin cobalt binding

Aims of study:
To monitor the dynamics of albumin cobalt binding (ACB) prior to a percutaneous coronary intervention (PCI) and at predefined time points after PCI. To determine whether development of myocardial necrosis in PCI, as documented by troponin I (TnI) positivity, will have an effect on ACB.
Method and patient group:
A group of 61 patients was divided into two subgroups: (1) patients after diagnostic coronary angiography; (2) patients undergoing PCI. Using spectrophotometry, ACB was determined in either subgroup at time 0
(placement on the catheterization table) and at times 30, 60, 120, and 360 minutes. All measured values were also adjusted for mean albumin value. Mean ACB values at individual time points were mutually compared. The subgroup of five post-PCI patients with TnI positivity was evaluated separately. Their baseline (time 0) ACB levels were compared additionally with those of controls (32 patients with repeatedly physiological ECG curve, repeatedly normal TnI and myoglobin levels, and/or normal coronary angiography findings).
Results:
Mean baseline ACB levels obtained in both subgroups did not differ from those seen in controls. Marked spread of values obtained from blood sampling at identical time points was observed in all (sub)groups. The spread of values was smaller after adjustment of ACB to albumin levels. No significant difference was seen in ACB levels between groups with and without PCI, or in the subgroup with clear evidence of previous myocardial ischemia. More detailed analysis showed there was also no correlation between the magnitude of myocardial necrosis (reported TnI levels) and ACB levels.
Conclusion:
Based on our results, albumin cobalt binding does not seem to contribute to establishing the diagnosis of myocardial ischemia.
