Optimal localization of the electric lead system in cardiac resynchronization therapy

Cardiac resynchronization therapy has become an integral part of management of patients with advanced chronic heart failure and cardiac dyssynchrony defined by QRS complex widening in the guidelines. Conceptually, the method is based on mechanical resynchronization of asynchronous, antrioventricular, interventricular, and left intraventricular contractions. A major determinant of resynchronization therapy outcome is the position of individual leads, specifically the left ventricular, right ventricular and, theoretically, also the atrial leads. The review addresses various aspects of identifying the optimal position for individual leads as well as possibilities of assessing the lead system for resynchronization therapy in individual patients in clinical practice.
