Srdeční selhání (a nejen srdeční selhání) a fibrilace síní: rozevírající se nůžky mezi důkazy a praxí

Fibrilace síní (FS) se stala velkým medicínským a socio-ekonomickým problémem. Obnovení a udržení sinusového rytmu je sice spojeno s vyšší kvalitou života a delším přežíváním, ale dosažení tohoto cíle je v praxi obtížné. Článek se zabývá hodnocením role antiarytmik, ablace AV junkce a kardiostimulace, selektivní ablace a také jejich přímým srovnáním v léčbě FS a srdečního selhání. Zabývá se také problematikou elektrické, mechanické a strukturální remodelace a případné reverzní remodelace myokardu a potenciálem jednotlivých metod tyto děje ovlivnit. Dále se v článku hodnotí výskyt tromboembolických komplikací po selektivní ablaci ve srovnání s rutinní antikoagulační léčbou.
