Introduction: This study investigated the relationship between the subjective New York Heart Association (NYHA) class and objective exercise capacity (VO2peak) in patients with heart failure and whether it is influenced by medication adherence. Methods: Data from 170 heart failure patients in the Level-CHF register were analysed, and adherence was assessed by serum drug level testing. Objective exercise capacity was assessed by cardiopulmonary exercise testing using a bicycle ergometer. Results: Objective VO2peak frequently exceeded the subjective NYHA class with 46% concordance, especially at the extreme ends of the spectrum....

Pulmonary edema in swimmers and divers is defined as sudden onset of shortness of breath accompanied by cough, hemoptysis, and hypoxemia that occurs in association with increased physical activity, especially in a water/cold environment. It occurs more often in women and in the elderly, predisposing factors include a cold environment and the presence of some latent cardiovascular diseases. It is a potentially life-threaten- ing condition for athletes. Its therapy does not differ from the treatment of pulmonary cardiac edema. Early identification of persons with a higher risk and elimination of all potentially risk factors for its occurrence is very...

Secondary prevention through comprehensive cardiac rehabilitation has been recognized as the most cost-effective intervention to ensure favourable outcomes across a wide spectrum of cardiovascular disease, reducing cardiovascular mortality, morbidity and disability, and to increase quality of life. The delivery of a comprehensive and 'modern' cardiac rehabilitation programme is mandatory both in the residential and the out-patient setting to ensure expected outcomes. The present position paper aims to update the practical recommendations on the core components and goals of cardiac rehabilitation intervention in different cardiovascular conditions,...
