Background: To assess the distribution of costs associated with Cardiology Unit hospitalization due to acute heart failure (AHF) and evaluate, from the perspective of the healthcare payer, the heterogeneity of resources use according to AHF etiology in patients from 2005 to 2009.Methods: The type and etiology of AHF was determined upon hospital admission. The cost of in-patient care was based on the individual hospital account of each patient (1759 patients in total; 58.7% male; mean age 71 years). Results: The median hospital stay was 7 days and the mean total cost of in-patient care was €3364. A Coronary Care Unit (CCU) stay was recorded...

Aims: This study analyzed the arrhythmogenic substrates and mechanisms of ventricular tachycardia (VT), and long-term outcomes of catheter ablation in patients with arrhythmogenic right ventricular cardiomyopathy/dysplasia (ARVC/D). Methods: Nine patients (1 female, 40 ± 17 years) with ARVC/D and sustained monomorphic VT (SMVT) exhibiting left bundle branch block morphology of the QRS complex were studied. The diagnosis of ARVC/D was confirmed by means of echocardiography, magnetic resonance imaging, and electroanatomical mapping in all patients. Results: The patients underwent 10 ablation procedures. At the initial...

Cardiac amyloidosis: A comprehensive review

Michal Fikrle, Tomáš Paleček, Petr Kuchynka, Eduard Němeček, Lenka Bauerová, Jan Straub, Romana Ryšavá

Cor Vasa 2013, 55(1):e60-e75 | DOI: 10.1016/j.crvasa.2012.11.018

Cardiac amyloidosis is characterized by clinically significant extracellular amyloid infiltration of the heart that is usually, but not always, associated with the involvement of other organs depending on the type of amyloid. Cardiac involvement represents the most important prognostic factor especially in AL amyloidosis and thus early diagnosis of amyloid heart disease is of utmost importance influencing further management of the patients. This review aims to broadly discuss pathogenesis, manifestation and complex diagnostics of amyloidosis with the main focus on amyloid cardiomyopathy. Also, the summary of current therapeutic options that have great...

Cor Vasa 2013, 55(1):e82-e84 | DOI: 10.1016/j.crvasa.2012.10.001

The recently updated Universal Definition of Myocardial infarction (MI) arbitrarily defined periprocedural MI (type 4a) by elevation of cardiac troponin (cTn) values > 5× the upper reference limit (URL) in patients with normal baseline values or a rise of cTn values > 20% if the baseline values are elevated, together with either angina or new ECG changes or angiographic loss of patency of a coronary artery or a side branch or persistent slow or no-flow or embolization, or imaging demonstration of new loss of viable myocardium.Although an association between CK-MB elevations and adverse prognosis after PCI has been documented, existing data...

Left ventricular perforation by a guidewire crossing a stenotic calcific aortic valve

Şenol Demircan, Hakan Altay

Cor Vasa 2013, 55(1):e76-e77 | DOI: 10.1016/j.crvasa.2012.05.016

With the availability of better technology in catheterization techniques, perforation of myocardium during cardiac catheterization is a rare event. We report, a 65-year old male who developed this potentially life threatening complication, after several attempts were made to cross a heavily stenotic aortic valve. With this case presentation, we aim to call attention to this complication which we think will occur more frequently in the era of percutaneous aortic valve replacement.
