Inflammatory cardiomyopathy is a term used for myocarditis associated with left ventricular dysfunction. There are many infectious and non-infectious causes of this disease with viral infection being the most common. Even though the efficacy of acetylsalicylic acid (ASA) and clopidogrel has been documented in patients with acute and chronic coronary artery disease (CAD), many patients develop thrombotic cardiovascular complications despite dual anti-platelet therapy due to nonresponsiveness or partial responsiveness. Monitoring the dynamics of B-natriuretic peptide may have a major impact on the choice of treatment of the patient with heart failure

The paper seeks to provide evidence that repeat measurement of serum B-natriuretic peptide levels may contribute to deciding on the therapeutic option to be chosen for the heart failure patient. We report the case of advanced heart failure in a patient with spongiform cardiomyopathy placed urgently on a waiting list for heart transplantation. The ensuing course was unexpectedly favorable and the patient is still being treated conservatively. A decrease in B-natriuretic peptide levels served as an early marker of resolving signs of advanced heart failure.

Pulmonary arterial hypertension (PAH) is generally known to be a hereditary disease; it is believed to develop predominantly in younger individuals. In fact, the diagnosis of PAH is usually established later, when patients are about 41-60 years old. The main reason for the late diagnosis is the non-specific presentation of the disease. Additional reasons may include the short time elapsed from the definition of the diagnosis and the fairly poor therapeutic options available until recently, explaining the shift of detection and treatment of the disease towards elderly patients.
