Jan Bruthans, Otto Mayer jr., Jaroslav Šimon, Markéta Galovcová, Jana Hrbková, Jiří Bělohoubek, Monica Patraulea, Jiří Jeschke, Hana Rosolová, Renata Cífková

Cor Vasa 2008, 50(12):446-454 | DOI: 10.33678/cor.2008.167

Aims of study: To assess the prevalence of major risk factors for ischemic stroke, to determine implementation of the secondary prevention of stroke in clinical practice. Methods: A total of 507 consecutive patients aged < 80 years and hospitalized for their first ischemic stroke in the respective district hospital (Praha 4 and Plzeň-město) were selected retrospectively. Data were obtained from patients' hospital medical records and subsequent patients' history including the completion of structured questionnaires. Outpatient examination including fasting blood sampling was performed at 6-36 months post-stroke. The survey was carried out as part...

Coronary artery ectasia

Jiří Knot, Josef Bis, Richard Rokyta Jr., Josef Šťásek, Petr Widimský

Cor Vasa 2008, 50(12):455-459 | DOI: 10.33678/cor.2008.168

Background: Localized or diffuse dilatation of the coronary arteries (coronary ectasia) is a well-known, albeit rare, angiographic finding. Its incidence ranges from 0.2% to 5.3% in the literature. We aimed to determine its incidence, baseline characteristics, and prognosis in patients in the Czech Republic. Methods and Results: All 6,997 consecutive coronary angiograms performed through 12 months in three cardiac centers in the Czech Republic were analyzed. The follow-up period was one year. Of the 6,997 patients, only 52 (0.7%) were assigned to the coronary artery ectasia group. The right coronary artery was the most common vessel affected by...

The role of microRNAs in cardiovascular disease

Martin Pěnička, Karel Vondrák

Cor Vasa 2008, 50(12):460-463 | DOI: 10.33678/cor.2008.169

Recent experimental work has highlighted the enormous importance of what is referred to as microRNAs in the post-transcriptional regulation of gene expression. MicroRNAs are single-stranded RNA molecules of about 22 nucleotides in length belonging to the family of non-coding RNA. MicroRNAs induce a decrease in "messenger" RNA thereby inhibiting protein synthesis at translation level. The only specific type of microRNAs regulates the expression of a whole chain of genes encoding functionally or structurally related proteins. Experimental work has identified microRNAs as key regulators of genes affecting the growth and hypertrophy of cardiomyocytes,...

Current alternatives to anticoagulation therapy in patients with atrial fibrillation

Miloš Táborský, Petr Neužil

Cor Vasa 2008, 50(12):465-469 | DOI: 10.33678/cor.2008.170

Atrial fibrillation is the most common type of supraventricular arrhythmias. Based on formation of atrial trombi, especially in the left atrial appendage, it is associated with a significantly increased risk for developing stroke. Different medical, surgical and catheter-based methods for stroke prevention in patients with atrial fibrillation will be discussed.

Ischemic and non-ischemic myocardial injury: differential diagnosis using post-contrast delayed magnetic resonance imaging scans

Josef Vymazal, Jiří Weichet, Jan Balák

Cor Vasa 2008, 50(12):470-472 | DOI: 10.33678/cor.2008.171

The rapid development of magnetic resonance imaging (MRI) has had an impact also in cardiology and cardiac surgery. Use of delayed scans following contrast agent administration allows to detect myocardial scars, distinguish subendo-cardial from transmural injury and, also, to detect non-ischemic myocardial injury. While non-ischemic myocardial injury tends to be central and non-coalescent, ischemic myocardial injury is either subendocardial or transmural in nature, very often combining both types, whereby a transmural scar turns into a subendocardial lesion. The ischemic nature of injury respects vascular territories. Non-ischemic myocardial injury,...

Use of CT angiography and magnetic resonance imaging in cardiology

Michael Aschermann

Cor Vasa 2008, 50(12):473-479 | DOI: 10.33678/cor.2008.172

CT angiography and magnetic resonance imaging play an increasing role in the diagnosis of cardiovascular disease. The paper summarizes main concepts regarding the current status of development of computed tomography and -magnetic resonance imaging technology; it also discusses the potential for indicating these techniques of examination in clinical cardiology. A summary of concepts regarding the potential risk of radiation associated with CT angiography and magnetic resonance imaging is included.

The TIMI trials - 25 years of clinical research (1984-2009)

Jindřich Špinar, Jiří Vítovec, Lenka Špinarová

Cor Vasa 2008, 50(12):480-485 | DOI: 10.33678/cor.2008.173

The TIMI group has celebrated 25 years of its existence and 50 clinical trials conducted during the American Heart -Association (AHA) Annual Meeting held in New Orleans in November 2008. The TIMI group was founded in 1984, with Professor Eugene Braunwald being the leading cardiologist of the group since the very beginning. TIMI has evolved into a wide array of phase I to phase IV trials as well as registries. These have ranged in size from less than 30 to more than 300,000 subjects. The TIMI group has formulated several definitions in cardiology, e.g., the myocardial infarction definition, TIMI bleeding definition, TIMI flow grade (perfusion grade)...
