Cor Vasa 2007, 49(10):356-361 | DOI: 10.33678/cor.2007.128

Background/Aims:To study the perioperative and postoperative serum levels of heat shock proteins Hsp60 and Hsp70 in cardiac surgical patients operated with or without cardiopulmonary bypass (CPB). Methods:Thirty-four patients undergoing coronary artery bypass grafting (CABG) were operated either with or without the use of CPB. The subgroups did not differ in age, sex, number of anastomoses, and ejection fraction. Samples of venous blood were taken at five intervals: at the induction to anesthesia, upon completion of surgery and on postoperative days 1, 3, and 7. Hsp60 and Hsp70 levels were detected by commercially available...

Prevalence and clinical characteristics of resistant hypertension in a specialist center

Tomáš Pikus, Jiří Widimský jr., Tomáš Zelinka, Jan Rosa, Branislav Štrauch, Ondřej Petrák, Robert Holaj

Cor Vasa 2007, 49(10):351-354 | DOI: 10.33678/cor.2007.127

Aim of study:To determine the prevalence and clinical characteristics of resistant hypertension in a group of patients examined in a hypertension center. Methods:We examined a series of 620 individuals (mean age, 55 ± 10 years) with arterial hypertension referred to our Hypertension Center over the years 2005-2006 in an effort to assess the possibility of a secondary etiology of the hypertension. During hospitalization, all patients had a thorough diagnostic assessment including determination of selected hormonal parameters, 24-hour ambulatory blood pressure monitoring, and use of imaging techniques. Resistant hypertension...

Diuretics in the treatment of hypertension and the risk of new-onset diabetes

Jiří Widimský

Cor Vasa 2007, 49(10):362-367 | DOI: 10.33678/cor.2007.129

A recent meta-analysis of studies of treatment of hypertension has demonstrated that new-onset diabetes develops least often during therapy with angiotensin II receptor type 1 blockers (ARBs) and ACE inhibitors. However, these data do not make it clear whether the agents modulating the renin-angiotensin system exert a direct antidiabetogenic effect or whether they are only devoid of the diabetogenic action of beta-blockers and diuretics. The DREAM study, however, failed to show whether high doses of the ACE inhibitor ramipril, as compared with placebo, would effectively prevent the development of diabetes. The relative reduction in cardiovascular...

Left ventricular pseudoaneurysm as a complication of myocardial infarction

Unlike true aneurysm, left ventricular pseudoaneurysm (PSA) as a complication of myocardial infarction occurs quite rarely. Its exact prevalence is not known, with data regarding the diagnosis, treatment and natural history of pseudoaneurysm obtained only from individual case reports. Judging by data available, the risk for rupture is high (30-45%), particularly within the first three months of a pseudoaneurysm developing. The most frequent causes of death in the long term are electromechanical dissociation and sudden death. Left ventricular PSA may be difficult to diagnose, especially because of its localization, occurring as it does most often in...

Cardiac tumors

Cardiac tumors (CTs) are divided into primary and secondary ones. Secondary CTs occur 20-40 times more often compared with primary CTs. The paper presents our two case reports of cardiac malignancy. The first case involved uterine cancer via intracavitary metastasis of the right ventricle recurring at 11 years after hysterectomy and radiotherapy. The second case was right atrial angiosarcoma. We find the cases noteworthy because of their diverse history, similar clinical presentation (embolism into the pulmonary vessel bed and dyspnea) and the much different outcome of the two female patients. The patients were examined using available techniques,...

A case report of transient left ventricular apical ballooning

Vladimír Kaučák, Jan Mrózek, Petr Černý

Cor Vasa 2007, 49(10):378-382 | DOI: 10.33678/cor.2007.132

Transient left ventricular apical ballooning (also known as emotional myocardial stunning, Tako-tsubo syndrome, ampulla cardiomyopathy) is a relatively novel, not well-characterized syndrome of transient systolic left ventricular (LV) dysfunction, characterized by extensive systolic LV dysfunction involving dyssynergy (akinesia or even dyskinesia) of apical to mid-LV segments without significant coronary stenosis, and with clinical presentation mimicking acute coronary syndrome. It predominantly affects elderly females and is preceded by emotional or physical stress, or aggravation of a serious somatic disease. In this case report, we describe the...
