Radiofrequency thoracic sympathetic denervation a new option in the treatment of ischemic upper limb conditions

Background:The aim of this pilot study was to evaluate the success rate of percutaneous radiofrequency thermolesion in the treatment of ischemic upper limb conditions. Methods:Twenty selected patients underwent radiofrequency thermolesion of the upper thoracic sympathetic chain using controlled lesion at the level of T2 and T3. The success rate of the procedure was evaluated using both subjective andobjective methods as well as the frequency of complications. Background and methods:These are described in the first part of our article (CV 2006;48/12/:433-440). Results:Unfractionated heparin (UFH) was administered to 76 patients (55.8%), low-molecular weight heparin (LMWH) (enoxaparin) s.c. to 55 patients (45.8%). Because of contraindications or bleeding complications, 18 patients (15%) could not receive anticoagulation and/or anticoagulation had to be discontinued. These patients had appreciably higher mortality rates (50%). Thrombolysis was administered to 14 (11.7%) patients, a vena cava inferior venous filter was implanted in 15 patients (12.5%), 19 patients (15.8%) were hospitalized...

Unlike pulmonary arterial hypertension, pulmonary hypertension in chronic lung diseases is defined by a mean pulmonary artery pressure ≥ 20 mmHg (being ≥ 25 mm Hg in pulmonary arterial hypertension) and a capillary wedge pressure below 12 mmHg. Cardiac pacing in some Prague-based hospitals in recent years

Studies conducted in recent years have shown that double-chamber cardiac pacing, as compared with single-chamber cardiac pacing, in patients with sick sinus syndrome and major atrioventricular conduction disorders, does not decrease mortality or the incidence of cardiac insufficiency. The ratio of primary implantations of double-chamber and single-chamber pacemakers in large Prague-based hospitals in 2004 and 2005 varied over a big range of 3.4 to 0.2. The difference can hardly be solely explained by the specialization of centers. Implantation criteria should be revised and their implementation reviewed.
