Aim:To assess the effect of switching patients undergoing orthotopic heart transplantation (OHTx) with recurrent cellular rejection episodes from cyclosporin A (CyA) to tacrolimus (TAC). Method:We followed a group of 17 OHTx patients (5 women, 12 men, age range 53 11 years), switched to TAC in 2001-2005 for repeat findings of higher grades of acute rejection episodes in endomyocardial biopsy (EMB) samples. We compared the incidence of acute rejection episodes in a total of six EMB samples from each patient, with each three obtained prior to and after switching the patient to TAC. Each grade of the Banff classification was...
Endoscopic saphenous vein harvesting for coronary artery bypass grafting. Postoperative and mid-term outcome in a group of 100 patients

Cor Vasa 2006, 48(12):427-432 | DOI: 10.33678/cor.2006.136
Background:Traditional great saphenous vein harvesting for coronary artery bypass grafting is associated with a significant risk of impaired wound healing. Endoscopic saphenous vein harvesting (EVH) has been introduced in an effort to reduce the incidence of leg wound complications, whereas the histological quality and long-term patency of harvested grafts are comparable with those of grafts harvested using the traditional technique. Methods:From July 2005 through March 2006, EVH was performed, using the Vasoview 6™ system (Guidant Europe S. A., Belgium), in a group of 100 patients. Patients were evaluated for impaired...

Cor Vasa 2006, 48(12):433-440 | DOI: 10.33678/cor.2006.137
Background:The aim of our study was to assess the incidence of pulmonary embolism (PE) in an unselected group of patients hospitalized in the department of medicine of a community hospital, to find out the incidence of risk factors, signs and symptoms of PE, to evaluate the accuracy and the benefit of paraclinical examinations, to analyze the therapeutic approaches, mortality and its risk factors. We compared our group of patients with the group of patients hospitalized for PE in 1996-1998. Methods:We analyzed the medical records of 120 patients hospitalized between 1 November 2001 and 30 June 2004 and diagnosed to have PE....

Left ventricular remodelling adaptation or maladaptation?
Radovan Jirmář, Václav Pelouch, Petr Widimský
Cor Vasa 2006, 48(12):441-446 | DOI: 10.33678/cor.2006.138
The paper summarizes current concepts regarding the development, progression and clinical relevance of left ventricular remodelling at cellular and subcellular levels. Also addressed are some signal processes critical for the development of left ventricular remodelling and optional therapeutic interventions. Special attention is given to changes occurring in the myocardial interstitium during the process of remodelling.
