Aim of study:To determine the status of bone mass, biochemical parameters of mineral metabolism and markers of bone remodeling in heart transplant recipients. Group of patients and method:The group included 45 heart transplant recipients. We investigated the main biochemical markers of mineral metabolism in the serum (Ca, Mg, Pi, total ALP) and urine (Ca, Mg, Pi and their fractional excretion rates) of patients. Renal function was determined using creatinine clearance. The levels of intact parathyroid hormone (iPTH) were determined by IRMA; those of osteocalcin using ELISA and urinary N-telopeptide (NTx) by EIA. 25-OH vitamin...

Aim:To assess the clinical importance of the phenomenon of apoptosis following ischemia-reperfusion caused by cardioplegic myocardial arrest in cardiac surgery and its modulation by sevofluran. Method:A prospective clinical trial in 40 patients (20 + 20) undergoing standard off-pump cardiac surgery procedure in extracorporeal circulation (Capiox membrane oxygenator, St. Thomas' cold cardioplegia, normothermia with local cooling). One group was receiving sevofluran, an inhaled anesthetic enhancing myocardial resistance to ischemia, throughout the procedure. Myocardial tissue specimens (3× 3 mm) were removed from the right atrial...

The effect of a rehabilitation program on the prognosis of myocardial infarction survivors
Václav Chaloupka, Lubomír Elbl, Svatopluk Nehyba, Iva Tomášková
Cor Vasa 2004, 45(1):29-35
Aim of study:To assess, in long-term follow-up, overall and cardiovascular mortality and the incidence of cardiovascular events (reinfarction, revascularization) in myocardial infarction (MI) survivors undergoing a rehabilitation program. Another objective was to assess the prognostic value of some clinical and exercise markers. Patients and methods:Patients after a myocardial infarction or a revascularization procedure for angina pectoris were included into a two-month outpatient program. Patients had three exercise sessions per week in groups of 4-6 persons. The total number of patients is 582 (89% of men and 11% of women),...

Chylothorax: a rare complication following cardiac surgery
Daniel Čoček, Marek Šetina, Adolf Drs
Cor Vasa 2004, 45(1):36-37
Chylothorax is a rare complication in cardiac surgery. We present the case report of a 63-year-old female patient undergoing myocardial revascularization with this particular complication, which was successfully managed conservatively.

Diastolic heart failure and CHARM
Špinar J, Vítovec J
Cor Vasa 2004, 45(1):38-40
Chronic heart failure occurs in 0.4-2% of the European population, with a prevalence of 1.3% reported for Eastern Europe; one out of three cases are diagnosed as diastolic dysfunction. Diastolic dysfunction is defined as a condition whereby there the ventricle is not filled enough, in the presence of normal left atrial pressure, to provide for adequate cardiac output. The main causes of diastolic dysfunction include coronary heart disease, hypertension, hypertrophic cardiomyopathy, heart valve disease with myocardial hypertrophy, restrictive myocardial disease, and constrictive pericarditis. The goal of diastolic heart failure management is to improve...
