Caseous calcification of the mitral annulus

Caseous calcification of the mitral annulus is a rare form of mitral annular calcification which may often cause diagnostic quandary. We report the case of a 69-year-old man scheduled for cardiac surgery on suspicion of a malignant process involving the posterior mitral annulus and encroaching upon the left atrium and left ventricular myocardium. During surgery, a caseous calcification of the mitral annulus was detected; a large quantity of white, toothpaste-like, caseous material was obtained, eliminating the suspicion of a tumour. Following surgery, motility of the posterior mitral valve leaflet was regained and the mitral valve gradient decreased from 6 mmHg to 2 mmHg. The surgical procedure was uneventful.
